Additional $20 Million Directed to Rural Telehealth Expansion

It has been well documented that the COVID-19 pandemic resulted in unprecedented increases in telemedicine utilization across the U.S. However, rural providers and patients, as evidenced by their lower rates of telemedicine usage during this time, have not been able to take advantage of the opportunities provided by telemedicine to the same extent as urban providers.

On August 18, 2021, the Health Resources and Services Administration (HRSA) of the Department of Health and Human Services (HHS) announced the latest attempt to ameliorate this issue – the distribution of nearly $20 million to 36 recipients for the purpose of strengthening telehealth services in rural and underserved communities and expanding innovation and quality. Bezos is said to have a fairly long-standing interest in longevity research. But although Altos Labs has so far managed to recruit some impressive names in the biological reprogramming sector, as well as some pretty big investors, its research still needs a lot of work before it can ever be applied to humans.

What is DAO - Decentralized Autonomous Organizations

A decentralized autonomous organization (DAO), sometimes called a decentralized autonomous corporation (DAC), is an organization represented by rules encoded as a computer program that is transparent, controlled by the organization members and not influenced by a central government. A DAO’s financial transaction record and program rules are maintained on a blockchain. The precise legal status of this type of business organization is unclear.

A well-known example, intended for venture capital funding, was The DAO, which launched with $150 million in crowdfunding in June 2016, and was nearly immediately hacked and drained of US$50 million in cryptocurrency. The hack was reversed in the following weeks, and the money restored, via a hard fork of the Ethereum blockchain: the Ethereum miners and clients switched to the new fork.

Similarly, Facebook’s data privacy practices do not increase people’s confidence in the company’s products. All the scandals that have surrounded the social network could backfire when Facebook wants to step into healthcare – and this is exactly what we wrote about in our latest article, Is There A Place For Facebook In Healthcare? In it, we looked at what Facebook currently does in medicine and evaluated whether those are viable ways to follow in the future.

COVID: The Health Economics of OTC Home Tests

What is an at-home Covid test?

There are two types of tests for COVID-19. Viral tests tell you if you have a current infection, and antibody tests tell you if you’ve been previously infected.

If you’re experiencing symptoms or think you’ve been exposed to COVID-19, contact your health care provider or your state or local public health department to find out where you can get tested. Tests are available at many health centers and some pharmacies. Call in advance to see if an appointment is required. The testing process and timeline for results vary by location.

But – Rather than having a doctor or health professional get all up in your nostrils, you can swab yourself and get the results in less than an hour. At-home rapid tests (known as “antigen” tests) are less reliable than the lab-based PCR [polymerase chain reaction] test, but experts say they can be an extremely useful tool for allowing life to proceed semi-normally.

NOTE: PCR means polymerase chain reaction. It’s a test to detect genetic material from a specific organism, such as a virus. The test detects the presence of a virus if you have the virus at the time of the test. The test could also detect fragments of the virus even after you are no longer infected.

Problem is, in the US over-the-counter rapid tests are expensive and scarce.

Abbott Laboratories sells a two-pack for $24, and Quidel’s QuickVue sells a test for $15. But even if you are willing to shell out for one, good luck finding a rapid test on pharmacy store shelves or on e-commerce websites, where they’re often sold out.

“Smaller companies [including dental offices] are learning that, as more data is shared online, they, too, can be targets for the kinds of attacks that larger firms endure. American International Group Inc. and Travelers Cos. are among insurers tailoring cybersecurity products to those customers.”

The Expert Speaks

Bob Parisi, network security and privacy practice leader at the insurance brokerage of Marsh & McLennan tells DelawareOnline that small and mid-size companies are “where we’re going to see some of the most aggressive growth in the next couple of years, because it’s been a part of the market that was ignored.”

The ad describes how a California-based online print shop was targeted by hackers who exposed clients’ names, addresses and credit-card numbers last year. Much like dentists whose EDRs are hacked, after discovering the breach, business owner David Handmaker had to notify affected customers. The Ponemon Institute predicts that 20% or more of the customers notified will instantly become former customers.

“We’re just much, much more aware of the fact that being a small company” makes us more of a target,” Handmaker tells DelawareOnline. He adds that larger businesses have “more resources, and so I think their security practices are maybe a little more evolved.”

Assessment

Small businesses such as print shops and dental practices have become de-facto targets – and according to security experts, easy pickings. I’m not wrong. I’m early.

The paperback book is small and compact in size but amazingly full of words, abbreviations, and even names of leaders in the health information technology industry. While any book like this will require updating on a periodic basis, many of the terms will remain relevant for a good period of time. I found the dictionary very useful and recommend it as a good addition to the reference shelf in the office or library.

That big medical news was announced Saturday by the biotech startup Intellia Therapeutics and its partner Regeneron, which said their gene-editing techniques reduced the amount of harmful liver protein associated with a genetic nerve disorder.

What is CRISPR? It stands for “clustered regularly interspaced short palindromic repeats,” and it’s one of those things humans found in nature and then copied. Bacteria use CRISPR to repel viruses, but humans have harnessed it to ctrl+c, ctrl+v DNA sequences, potentially leading to a revolution in treating disease. The two scientists who made that breakthrough in 2012, Jennifer Doudna and Emmanuelle Charpentier, won the Nobel Prize in Chemistry last year (Doudna is also a cofounder of Intellia).

And there’s not many moments in medicine where you get to experience that,” Intellia CEO John Leonard said. Looking ahead…expect Intellia shares, which have gained 233% since its 2016 IPO, to pop.

MEDICAL: Artificial Intelligence in EHRs

Epic Systems, the country’s leading e-health record company, says an algorithm it developed can accurately flag sepsis in patients 76% of the time. The life-threatening disease, which arises from infections, is a major concern for hospitals: One-third of patients who die in hospitals have sepsis, per the CDC.

Generally, the earlier sepsis is diagnosed and treated, the better a patient’s chances of survival—and hundreds of hospitals use Epic Systems’s sepsis prediction model, The Verge reports.

The problem: According to a study published this week in JAMA Internal Medicine, Epic Systems may have gotten the success rate wrong: The model is only correct 63% of the time—“substantially worse than the performance reported by its developer,” the researchers wrote.

Part of the issue can be traced to the algorithm’s development, Stat News reports. It was trained to flag when doctors would submit bills for sepsis treatment—which doesn’t always line up with patients’ first signs of symptoms.
“It’s essentially trying to predict what physicians are already doing,” Dr. Karandeep Singh, study author.

When reached for comment, Epic Systems told us the researchers’ hypothetical scenario lacked “the required validation, analysis, and tuning that organizations need to do before deployment,” adding that the JAMA study’s findings differed from other research.

We discuss the economic value of personalized medicine and the optimal pricing of the combination products involved. We build on previous work of Egan and Philipson (2015) who stress a link between rational adherence in health care and the value of personalized medicine. Personalized medicine converts experience goods to search goods by speeding up the learning process relative to trial and error. This explains the emergence of personalized medicine in cancer care as well as the timing of this emergence. It also predicts greater innovation-and merger incentives from Disneyland style two-part pricing of the combination products.

At a uniquely critical time in history, as dentists strive to keep their patients out of emergency rooms, pharmacists here in Fort Worth are telling dentists that the Texas State Board of Pharmacy has outlawed paper prescriptions – even for antibiotics.

According to three local pharmacists, on January 1, 2021 many dentists – including myself – will no longer be allowed to prescribe medications without digital records, no matter the urgency. As a dentist, I neither need nor want digital records, and as a patient, you shouldn’t either. Let me show you third-party nonsense which does NOTHING to improve patient care.

According to Protenus Breach Barometer, between April and June of 2017, 142 healthcare data breaches were reported, impacting 3.14 million records. 23 of the incidents involved paper, affecting 158,711 records. 158,711 divided by 3,140,000 = 5%.

This means that if your dentist put your identifying information on a computer, you had a 95% greater chance of medical identity theft than if your paper records were stored in a metal filing cabinet. In the last three years, the breaches have only worsened, cyber-crime is growing stronger every day, and most importantly, there are no solutions on the horizon. I ask you, why should any healthcare provider be forced by the state to knowingly risk Texans’ welfare?

Electronic dental records – both cloud-based and premises-based – are not only increasingly more expensive and increasingly more dangerous than paper for both dentists and patients, but they offer patients NO TANGIBLE BENEFITS over paper.

Just ask anyone.

But there’s more. Electronic dental records (which do nothing to improve care) also require far more training and specialized expertise than the working knowledge of alphabetical order required for filing charts in their correct place. Dental EHRs are first and foremost billing tools which not only shift the high cost of data entry from insurers back to dentists (read “patients”), but enable the really clever CEOs to control treatment decisions by employing strategic complexities for payment. All that digital offers dentists is convenience – expensive, dangerous convenience.

If Texans knew the truth, many would naturally prefer paper dental records. The business of dentistry is simply not so complicated that it requires computerization. After all, a dentist bills for treatments involving only the lower 1/3 of the face, and because dentistry involves intricate handwork, dentists can only safely treat a dozen or so patients a day – compared to 40 or more for physicians.

Very large, successful dental practices have thrived without computers for decades, while their patients enjoy the gold standard of security: Loud, heavy and cumbersome sheet metal filing cabinets.

On ZOOM Fatigue!

“Lately, Zoom meetings have been hitting a nerve with CEOs”.

JPMorgan Chase CEO Jamie Dimon says there’s no vital “creative combustion” happening in virtual settings. American Airlines CEO Doug Parker finds Zoom meetings awful.

And Microsoft CEO Satya Nadella calls them transactional, where “30 minutes into your first video meeting in the morning … you’re fatigued.

“What we as human beings need, want, seek … is human contact,” Nadella says.

Now members of the C-suite have gone full boomerang on Zoom meetings. After finding them awesome and productive at first, they’re now questioning how much they really achieve and are suggesting they lead to a sterile work culture lacking in imagination.

Dimon is particularly worried about how working from home has affected JPMorgan’s younger employees. He told analysts that productivity had dipped, especially on Mondays and Fridays. Dimon says bringing people back to the office is paramount to fostering creativity.

On August 3, 2020, President Donald Trump signed an executive order aimed at expanding access to care through two avenues: telemedicine and eased financial burdens on rural providers. The executive order builds on President Trump’s original expansion of coverage for telemedicine services in early March 2020, an order which was praised by the American Telehealth Association (ATA) and American Medical Association (AMA) for swiftly responding to the growing healthcare crisis. EMR ‘versus’ EHR

What is the Difference – Know the Difference

Both an EMR and EHR are digital records of patient health information. An EMR is best understood as a digital version of a patient’s chart. It contains the patient’s medical and treatment history from one practice. Usually, this digital record stays in the doctor’s office and does not get shared. If a patient switches doctors, his or her EMR is unlikely to follow.

By contrast, an EHR contains the patient’s records from multiple doctors and provides a more holistic, long-term view of a patient’s health. It includes their demographics, test results, medical history, history of present illness (HPI), and medications.

The “Chinee Room” Argument of A.I.

The Chinese room argument holds that a digital computer executing a program cannot be shown to have a “mind”, “understanding” or “consciousness”, regardless of how intelligently or human-like the program may make the computer behave.

The argument was first presented by philosopher John Searle in his paper, “Minds, Brains, and Programs“, published in Behavioral and Brain Sciences in 1980. It has been widely discussed in the years since. The centerpiece of the argument is a thought experiment known as the Chinese room.
