Markets: The NASDAQs 3-day rally fizzled as everything from Big Tech (Microsoft) to Smaller Tech (Snap) sold off.

On the other hand, Ford, one of the best performing companies in the S&P last year, notched a market cap of $100 billion for the first time in its 118-year history.

TODAY: Big banks will kick off Q4 earnings season.

PANDEMIC UPDATE: COVID-19 Prevention and Treatment

COVID-19 Prevention and Treatment

The majority of COVID-19 patients being treated in hospitals are unvaccinated or have not received a booster dose of the vaccine. These hospitalizations show how vaccines work and highlight the importance of continued vaccination efforts. President Biden has emphasized that vaccines, boosters, and therapeutic drugs have lessened the danger of COVID-19 for vaccinated people.  

  • Vaccine Recommendations: A new recommendation from the CDC says that people who were initially immunized with the Pfizer vaccine should now receive their booster dose after five months, down from six. Some people with weakened immune systems will soon be eligible for a fourth dose of the COVID-19 vaccine and an Omicron specific vaccine could be ready by March.
  • Vaccine Mandates: The Supreme Court is weighing the Biden administration’s requirement that workers get vaccinated or be tested regularly for COVID-19. Many states and cities have created their own vaccine rules and employers are left confused about what to do next during the legal battles and the current rise in cases.
  • At-Home Testing Shortage: There is a shortage of COVID-19 at-home test kits and kit prices are rising as are reports of price gouging. The White House announced a policy to make at-home tests freely available to Americans. Learn more about at-home tests and their accuracy here.
  • Antiviral Pills: Doctors express concern that the limited supply of antiviral drugs is unlikely to ameliorate the strain hospitals are experiencing. The U.S. doubled its order for Pfizer’s COVID pill so there’s enough for 20 million people.

UPDATE: Stock Markets and the Pandemic

  • Stock Markets: An inflation report couldn’t stop stocks from pushing higher yesterday, likely because it wasn’t worse than expected. Biogen shares tumbled after Medicare said it would limit coverage of its controversial $28,000 Alzheimer’s drug, Aduhelm; as the ME-P has noted.
  • Covid Pandemic: The current Omicron wave is projected to peak by January 19th in the US, according to an influential model from the University of Washington. Then, cases are expected to plummet “simply because everybody who could be infected will be infected,” Washington professor Ali Mokdad told the AP. Cases appear to have already peaked in Britain.

Earlier this week, GE’s Chairman and CEO Larry Culp shared a note with employees reflecting on GE’s priorities as we enter the new year. “Ten days into 2022, our work has already begun,” Larry said in his note. “I am excited for our road ahead.”

As we shared in November, GE is on a path to become three stronger, more focused companies positioned to lead in aviation, healthcare, and energy. Here are our priorities for the year ahead to take us there:

  1. Ensure safety is always first.

According to a recent study among 20,665 respondents at 124 institutions, intention to reduce hours were shown to be:

 •  Nurses: 33.7%
 •  Physicians: 31.4%
 •  Advanced practice providers: 28.9%
 •  Clerical staff: 13.6%
 •  Administrators: 6.8%

UPDATE: Markets, Pandemic, Economy and the IRS

  • Stock Markets: Down more than 2% with its back against the wall, the NASDAQ staged a huge comeback yesterday afternoon to close in the green and snap a 4-day losing streak.
  • Pandemic: Moderna was the S&P 500’s top performer after its CEO said that a booster shot targeting Omicron would soon enter clinical trials. Pfizer also said its Omicron booster would be ready by March.
  • Economy: A growing number of finance experts are taking the over when it comes to the number of interest rate hikes this year. Goldman Sachs now predicts the Fed will raise rates four times in 2022 (more than previously forecast) and JPMorgan CEO Jamie Dimon said he’d be surprised if it were only four hikes.
  • IRS: Even though tax filing season is just around the corner (opening January 24th with an April 18th deadline), the typically joyful and charismatic IRS has a case of the blues. On Monday, the Treasury Department warned that the agency has had a rough year and taxpayers should expect delays as returns are processed. According to Treasury officials, budget cuts and pandemic-related staffing shortages have created a towering backlog at the agency, and a “frustrating season” is on the horizon. While the IRS typically enters filing season with about 1 million unaddressed returns, the number stood at around 8.6 million in mid-Nov. 2021.

OMICRON VIRUS: Protect Yourself

The Omicron variant spreads more easily than the original virus that causes COVID-19. Here are 3 things you can do to help protect yourself and others:

  1. Get the COVID-19 vaccine, if you haven’t already. Vaccines are the best tool to protect us from COVID-19. They slow the transmission of the virus, and provide strong protection against severe illness and hospitalization.
  2. Get the booster when you’re fully eligible. Everyone 18 years and older should get a booster shot 2 months after their Johnson & Johnson vaccine, or 5 months after completing their primary COVID-19 vaccination series of Pfizer-BioNTech or Moderna. Adolescents and teens ages 12 to 17 should also get a booster of Pfizer-BioNTech 5 months after their primary series.
  3. Continue wearing a mask in indoor public places in areas of high or substantial community transmission. Use the COVID-19 County Check Tool to find your county’s level of community transmission.

UPDATE: The Markets

  • Markets: Stocks are off to a sputtering start in 2022, and they could be in for more upheaval this week with a big inflation report due, Fed Chair Jerome Powell’s confirmation hearing on Capitol Hill, and the beginning of earnings season.
  • NASDAQ: Last week, the tech-heavy NASDAQ fell 4.5% for its worst week since February 2021. And the ARK Innovation ETF, which is full of high-growth tech companies, plunged 11%.
  • Bonds: Over in the bond market, yields (or the return you can get from buying a bond) are surging. On Friday, the yield for the 10-year Treasury note hit its highest level since January 2020. Now, While rising yields are generally a bullish sign for the economy, they also make riskier assets—like expensive tech stocks—less attractive compared to other names that may get a boost from higher interest rates. The Dow, for example, with its many financial services companies, lost just 0.29% last week.

  • Stock Markets: The S&P is off to its worst start to a year since 2016, and the NASDAQ fell 4.5% this week—its worst drop since February 2021. The Fed’s hawkish pivot + rising bond yields are really pounding technology stocks.
  • Crypto-Currency: Bitcoin fell to its lowest level since last September, and other cryptos like ethereum and solana are also in the doghouse. It appears as though the Fed’s move to raise borrowing costs and the turmoil in mining powerhouse Kazakhstan are dragging down prices.

  • Markets: Already through a rough day, stocks dove even lower after the Fed released the minutes from its December meeting. Tech companies continued to get clobbered as rising bond yields make their shares less attractive.
  • About the Federal Reserve Minutes: Inflation anxiety was real at the central bank’s previous meeting, and officials signaled they could hike interest rates “sooner or at a faster pace” than previously expected to cool down prices.

What is an at-home Covid test? 

There are two types of tests for COVID-19. Viral tests tell you if you have a current infection, and antibody tests tell you if you’ve been previously infected.

If you’re experiencing symptoms or think you’ve been exposed to COVID-19, contact your health care provider or your state or local public health department to find out where you can get tested. Tests are available at many health centers and some pharmacies. Call in advance to see if an appointment is required. The testing process and timeline for results vary by location.

But – Rather than having a doctor or health professional get all up in your nostrils, you can swab yourself and get the results in less than an hour. At-home rapid tests (known as “antigen” tests) are less reliable than the lab-based PCR [polymerase chain reaction] test, but experts say they can be an extremely useful tool for allowing life to proceed semi-normally. 

NOTE: PCR means polymerase chain reaction. It’s a test to detect genetic material from a specific organism, such as a virus. The test detects the presence of a virus if you have the virus at the time of the test. The test could also detect fragments of the virus even after you are no longer infected.

Problem is, in the US over-the-counter rapid tests are expensive and scarce.

Abbott Laboratories sells a two-pack for $24, and Quidel’s QuickVue sells a test for $15. But even if you are willing to shell out for one, good luck finding a rapid test on pharmacy store shelves or on e-commerce websites, where they’re often sold out.

A Look at the FDA Drug Approval Process

It seemed too good to be true—on June 7, the FDA approved a breakthrough medication for Alzheimer’s disease. At the time, the drug “could mark a new era in treating the leading cause of dementia and the sixth leading cause of death in the US”, according to some experts.

–OR NOT–

The FDA’s approval of the drug was met with widespread criticism. And the ensuing scandal has undermined the integrity of the agency’s approval process for new treatments.  

Backlash to Aduhelm continued to escalate last week. On Thursday, influential hospitals Cleveland Clinic, NYC’s Mount Sinai Health System, and Providence in Washington State said they wouldn’t administer Aduhelm, citing concerns over its effectiveness and safety. 

Review 

According to Morning Brew, the FDA approving Aduhelm was like you deciding to watch a movie knowing that it had a 47% rating on Rotten Tomatoes. In November 2020, an expert panel advised the FDA against approving the drug after a pair of studies showed conflicting results about its effectiveness. 

Instead the agency fast-tracked it, arguing that the benefits of treating a devastating disease like Alzheimer’s outweigh the risks. An Alzheimer’s drug hadn’t been approved by the FDA in 18 years. 

Some experts who helped the FDA evaluate Aduhelm protested the decision. Harvard Medical School Professor Dr. Aaron Kesselheim, the third member of the 11-person advisory panel to resign in the wake of the FDA’s approval, said it was “probably the worst drug approval decision in recent U.S. history.”

Effectiveness is one concern around Aduhelm. Side effects, which include brain swelling and brain bleeding, are another. The final issue is the price tag: $56,000. That’s a lot, but it could cost even more since patients might have to pay for periodic tests that look for side effects.

A study by the healthcare nonprofit Altarum concluded that by the mid-2020s, Aduhelm would account for more than 1% of all national health spending, and that’s on the low end of projections. 

Looking Ahead … the blowback has been so bad that the acting FDA chief has ordered a federal investigation to explore the perhaps too-cozy relationship between the scientists who approved the drug and the drug-maker, Biogen. 

Still, “once the product is approved, the cat’s out of the bag, the horse is out of the barn,” Johns Hopkins Drug Safety Expert Dr. G. Caleb Alexander told the NYT. More than 600 sites across the US are getting ready to administer Aduhelm, and they’re expecting high demand.

Doctors love to drive. Nurses and medical professionals love to drive. We all love to drive!

But, what makes a state a great place for driving this Memorial Day weekend, 2021; especially post the corona pandemic? Is it higher gas prices, or fewer fellow travelers on the road? Or, is it higher speed limits–whether it’s on your way to a destination, or purely for the enjoyment of driving?

And, what makes other states among the worst places to drive?

We Want You to Know

Fear not! The ME-P and CarConnection looked at five sets of data to choose our best and worst states for driving. We selected the most recent data available from a range of sources–some neutral, some not so neutral. It was normalized so that it represented a percentage of change from the average, and added up the scores to determine which states earned the lowest total. The factors considered included the following.

  • Gas prices: The average price of a gallon of unleaded gasoline, on May 8, 2021, as surveyed by AAA, was $3.05
  • Miles driven per capita: From 2019 Department of Transportation data.
  • Speed limits: Highest legal speed limits posted by states, via the IIHS.
  • Accident rate: Per 100 million miles traveled, from 2019 NHTSA data via the Census Bureau.
  • Speeding tickets: Correlated by Motorists.org from Web searches for “speeding”-related terms, state by state.

Government Report Finds 92 Percent of Nursing Homes Employ Convicts

About 5 percent of nursing home workers—or one out of every 20—had at least one conviction, according to the report, which took a random sample of 260 nursing homes certified by Medicare and ran FBI background checks on their workers.

State Rules

State rules differ regarding background checks: 43 states require nursing homes to perform background checks against state records, ten of those require an additional FBI background check, and eight states don’t require background checks at all.

The rules also differ on what types of crimes disqualify workers. The report noted that of the workers with convictions, 44 percent had committed property crimes such as theft, vandalism or writing bad checks. Some 16 percent had drug-related crimes, and 13 percent had committed crimes against people, including sexual offenses.

Federal Regulations

Federal regulations prohibit nursing homes from employing workers convicted of “abusing, neglecting, or mistreating residents,” but because FBI data do not show whether the victims of the crimes were nursing home residents, it’s unclear whether these rules were violated.

The New York Times noted [2] that the current system for background checks—which Wisconsin Democrat Sen. Herb Kohl criticized as “haphazard, inconsistent, and full of gaping holes [3]—has allowed people convicted of crimes in one state simply find jobs at nursing homes in another state.

We’ve noted a similar lack of oversight in the nursing field, which allowed problem nurses to cross state lines in order to keep working and avoid consequences [4]. A national database was created decades ago to prevent this from happening, but reporting failures at both the state [5] and federal level [6] have left the database riddled with gaps.

CMS Over-Sight

The inspector general recommended that the federal government’s Centers for Medicare and Medicaid Services, or CMS, work with the states to develop background check procedures, including lists of convictions that would disqualify a potential hire.

CMS oversees nursing homes eligible for funding under Medicare and Medicaid. In a written response to the report, the agency agreed with the recommendation. CMS also runs Nursing Home Compare [7], a searchable database with ratings on nursing homes.

Richard Thaler wins 2017 Nobel Economics Prize

STOCKHOLM, Oct 9 (Reuters)

U.S. economist Richard Thaler won the 2017 Nobel Economics Prize for his contributions in the field of behavioral economics, showing how human traits affect supposedly rational markets, the Royal Swedish Academy of Sciences said on Monday.

Thaler brought to prominence the idea of “nudge” economics, where humans are subtly guided toward beneficial behaviors without heavy-handed compulsion, the theme of a 2008 book he co-wrote which caught the eye of policymakers around the world.

International Nurses Day is celebrated around the world every May 12th, the anniversary of Florence Nightingale’s birth.

The Florence Nightingale International Foundation

According to their website, the FNIF is the International Council of Nurses (ICN) premier foundation. It supports and complements the work and objectives of the ICN. Its history dates back to 1912 when a memorial to Florence Nightingale was first proposed by Mrs. Ethel Bedford Fenwick at an ICN Congress in Cologne. The vision was of an educational foundation for nurses. As the First World War intervened, it was not until 1929 that the memorial was finally activated by the ICN Grand Council in Montreal. Mrs. Bedford Fenwick, the first president of ICN was elected the first chair of the Florence Nightingale Memorial Committee.

History

From 1931, national Florence Nightingale committees were established in countries where ICN had member associations. In 1932 discussions were held between ICN and the League of Red Cross Societies to use their Red Cross international post-graduate courses as an international memorial to Florence Nightingale.

The ICN Grand Council agreed to take over the international courses, and in 1934, established the Florence Nightingale International Foundation (FNIF) as a living memorial to Florence Nightingale. FNIF became an autonomous organization under British law with its own governing body and a mandate to develop and promote nursing education world wide.

Charitable Intent

FNIF is a registered Charity in the United Kingdom and maintains its original purpose; to support the advancement of nursing education, research and services for the public good. Though UK registered, the day-to-day administration is carried out in Geneva through the International Council of Nurses, a federation of 133 national nurses’ associations working to ensure quality care for all and sound health policies globally.

FNIF is a new foundation for a new day! They have a new board, a new program and a new look. The focus is global – as the needs of nurses everywhere are similar but the resources to meet those needs differ around the world.

How did you get to work today? Did it improve your health?

When I asked a traffic engineer those questions, we ended up having a fascinating conversation. She was someone used to focusing on safety and was thrilled to consider the broader impact she could have on her community’s health.

In our quest to become the healthiest nation, these are the types of discussions we need to have more often. Today in America, your zip code can mean a 15 year difference in life expectancy. We can’t fix this in a doctor’s office. We need to work far upstream with housing, transportation, education, parks & recreation, energy, employment, criminal justice – all the areas of our society that unfortunately are not giving people an equal opportunity for a long and healthy life.

This position requires collaborative, communicative and transformational leadership and business skills.

About Regional Health

Regional Health is an integrated system of more than 40 entities, working  together to provide and support healthcare excellence in partnership with the communities they serve in western South Dakota. Rapid City Regional Hospital, a 417 bed facility, is the flagship for the system with 250 medical staff providing 23 areas of services ranging from Emergency Care, Intensive Care, and Stroke Care to Behavioral Health, Home Care, Wound Care, and Laboratory Services. The hospital also serves as the region’s  Level II Trauma Center. All members of the healthcare team of Regional Health share the mission of providing and supporting healthcare excellence in partnership with the communities they serve.

The Position

The Vice President of Medical Affairs has primary responsibility in a leadership position to affect quality outcomes by developing and strengthening physician relationships. The successful candidate will provide oversight and guidance in credentialing and privileging, physician peer review, and also the medical staff structure and function for quality assurance including medical staff bylaws and rules and regulations. This person provides key support for recruitment and retention of the most skilled, effective and ethical physicians in the market. The Vice President Medical Affairs will relate to the physicians as team members, will represent the physician perspective to the  administrative team and serve as a physician advocate promoting effective  physician/hospital/system relationships.

The Candidates

The successful candidate will have an MD or DO degree and be Board Certified in their specialty. The Vice President of Medical Affairs will have experience in Medical Staff management activities and in health care administration, with business administrative experience highly preferred.

Uber-Storm Notification from the Medical Executive-Post

All ME-P readers and subscribers; please be advised the National Weather Service is tracking Hurricane Sandy, a category 1 storm for potential land fall in the Delaware region this coming Monday evening. Tropical storm conditions with heavy rain and high winds are anticipated. The South will be affected less so.

[Vulnerable Areas]

Nevertheless, we would like to take this opportunity to assure you we are taking all necessary precautions to ensure our ME-P Atlanta Data Center maintains normal and uninterrupted service throughout this severe weather event. Our generator plant will be fully fueled, tested and ready to provide power to the facility during any long or short term utility outages.

Our datacenter operations, systems operations, and support teams will also be onsite closely monitoring the facility and appropriately prepared for extended duty to service our customers.

While we hope Hurricane Sandy misses us, we are taking all necessary steps to ensure we are adequately prepared. Stay safe!

2012 – Nobel Prize for medicine awarded to Gurdon and Yamanaka for stem cell discoveries

British scientist John Gurdon and Japanese researcher Shinya Yamanaka were jointly awarded the Nobel Prize in physiology or medicine yesterday, for experiments separated by nearly half a century.

US Treasury Secretaries Warn “Fiscal Cliff Ahead”

The Treasury Secretaries Opine

At a meeting in Washington on September 12, former Treasury Secretaries Robert Rubin and James Baker discussed the upcoming “fiscal cliff.” The forum was held in order to discuss the national debt and potential financial solutions. Treasury Secretaries from both parties were invited to discuss future financial options.

Both responded to the Congressional Budget Office (CBO) report that indicates the increased taxes and reduced spending scheduled to take place in January of 2013 could lead to recession. The Washington name for the potential financial challenge to the nation is “fiscal cliff.”

Rubin Speaks

Former Secretary Rubin stated, “It may be that for all kinds of reasons one party or another decides that’s the way to negotiate. But if we go over, we can’t stay over for very long, that’s for sure.” Rubin was referring to the effect of the potential fiscal cliff on negotiations between the parties in the November Congressional session.

Baker Speaks

Former Treasury Secretary James Baker joined by satellite; he stated, “There will be a recognition that we have to move the cliff out … so that we have time to deal with it and deal with it in an effective and orderly way, and I think that’s what’s going to happen.”

Secretary Baker was referring to proposals by Assistant Senate Majority Leader Richard Durbin (D-IL) and others. Durbin has introduced a plan to delay the scheduled changes for six months to enable the Senate Finance Committee and House Ways and Means Committee to develop and pass new legislation.

Boehner Speaks

House Speaker John Boehner (R-OH) was asked this week about the fiscal cliff and whether a compromise is possible. Boehner stated he is “not confident at all” that the House and Senate will be able to act. The House passed the Job Protection and Recession Prevention Act of 2012 (H.R. 8) to extend all tax rates for one year. The House also has passed the Pathway to Job Creation through a Simpler, Fairer Tax Code Act of 2012 (H.R. 6169) to set up a fast-track process for tax reform in 2013.

Assessment

However, the Senate has passed the Middle Class Tax Cut Act (S. 3412) and would increase taxes for upper-income individuals. Senate Majority Leader Harry Reid (D-NV) still indicated that he hopes for a compromise in November.

Did you know that recent reports show that nearly half of all Americans admitted to contributing to an unhealthy pool environment while even more are simply ignorant of the risks?

The Statistics

While, it is disgusting, it seems that 17% of adults admit to urinating in public pools.

So, during this Fourth of July Holiday, please take a minute to learn how to protect yourself and others this weekend, and summer.

More Yucky Info

20%, or 1  in 5 people pee in the pool.

Yep, that which we’ve always known to be true – because everybody secretly does it – has now been confirmed. People pee in the pool. One-in-five respondents to a survey by the Water Quality and Health Council admitted to contributing to the myriad disgusting contaminants already backstroking happily around the average pool.

Also, seven in 10 people don’t bother showering before diving in. Their sweat, sunscreen, dead skin, and other filth combines with chlorine to give you the red eyes and rashes you enjoy so much.

So next time someone goes all quiet while they’re kicking back in your pool, you might want to direct them to a bathroom.

I wanted to share our latest infographic with you, on organ donors and organ donation. Here are some of the highlights we uncovered:

  • Every 10 minutes someone is added to the National Transplant Waiting List.
  • Every donor can on average save up to 8 lives.
  • Every day an average of 79 people receive organ transplants and 18 people die waiting for transplants.

Celebrating Geek Pride Day

This glorious 25 May is once again upon us, towels are being folded correctly and thousands of voices are suddenly crying out in pain as they discover another little tweak to a certain film that was first released 35 years ago today. If that isn’t enough for you to celebrate, today is also once again Geek Pride Day. Last year IT recruitment agency Modis commissioned a survey of 1,000 Americans on their opinions around the term geek, and they’ve run it again this year to see how things have changed.

Smart Geeks

Looking through the results it seems that Geeks are becoming even more acceptable than last year, with 54% of respondents rating Geeks as “extremely intelligent” (up from 45% last year). 71% reckon we’re the people to go for technology advice (up from 56%), and more than half of Americans (51%) define Geeks as professionally successful – like medical professionals and ME-P readers – a huge leap from the 31% in last year’s survey. Around two-thirds of respondents defined a “Geek” as someone who is addicted to technology and who spends more time online than offline, yet when given a list of items and asked which they would have trouble living without, 71% of the “Geeks” put a pen and paper above technology and gadgets such as a computer (58%), smartphone (41%), or MP3 player (25 %). The non-geeks listed their cars as the thing they couldn’t live without!

Results

The results show that our addiction to our smartphones is still increasing with 45% of “Geeks” admitting to using their phones whilst driving – but it’s not clear if this is simply for sat-nav or musical reasons, or whether Angry Birds on the go is really taking off. (Note: please don’t be among this number – it’s a really silly, and illegal, thing to do). Slightly less worrying is using them in other inappropriate places such and weddings, funerals, on a date or, in 3% of respondents’ cases, at a job interview! Bet that went down well.

Surprises

One result we find surprising is that 60% of “Geeks” would find it more stressful to lose the data from their hard drives than to go through a relationship breakup. All we can say to that is that the pool of respondents couldn’t have contained too many GeekDr.Dads and GeekDr.Moms because; a) we would definitely miss our other halves more than a bit of data, and b) we’d have all that data backed up securely. Then again, the docs do seem to love their mobile aps.

Assessment

In several different locations, the ME-P and Modis is also running a little “GeekStakes” competition in conjunction with our friends at ThinkGeek over on their Facebook page until June 1st, where you can win some more gadgets to distract you from doing the thing that you’re supposed to be doing right now!

And so we ask – Doctors – Geeks, Nerds or NOT?
