What Do You Want?

It’s not easy, but the medical treatments you would want near the end of life need to be discussed with others. If you never bring up the topic and you were unexpectedly incapacitated and unable to speak for yourself, your medical wishes would never be known.

Despite the topic’s importance, only 27% of Americans report having talked with their families about end-of-life care. The best way to make your medical wishes known is to create an advance directive and share it with your family and your doctor.

Advance Directives

An advance directive is actually two legal documents that enable you to plan and communicate your end-of-life wishes.  When you create your advance directive, you are being proactive about your medical care and sparing your loved ones from having to make difficult medical decisions in a time of crisis.

What I Learned from Value Investor Guy Spier

A few months ago I was asked by the CFA Institute to give talks to CFA societies in London (October 27), Zurich (October 29) and Frankfurt (November 3). I enjoy giving occasional talks (but only a few a year, otherwise they become a chore). I also love Europe — history, old buildings and cultures, museums, sometimes a mild adventure. But this offer was much more interesting — I was asked to give a joint presentation with Guy Spier.

About  Guy

Guy Spier is a tremendous value investor who happens to be a good friend whose company I truly enjoy. He is the most cosmopolitan person I know. He was born in South Africa, spent his childhood in Iran and Israel, received his bachelor’s degree from Oxford and MBA from Harvard, lived in New York and in 2008 got sick of the New York hedge fund rat race and moved with his family to Zurich. His wife, Lory, is Mexican, so in addition to being fluent in languages of all the above-mentioned countries, he romances in Spanish.

Last year Guy published a book, The Education of a Value Investor: My Transformative Quest for Wealth, Wisdom, and Enlightenment. It is not a traditional investing book. In fact, I’ll say that differently: This is the most untraditional book on investing you’re likely to run into. It is a self-effacing memoir of Guy’s transformation from a Gordon Gekko wannabe who believes that his Ivy League education entitles him to Wall Street riches to a committed follower of Warren Buffett and his sidekick, Charlie Munger.

It must have taken a lot of guts and self-confidence (overcoming a lot of self-doubt) to write this book. To be honest, I am not sure I could have written it. It is one thing to strive for intellectual honesty; it is another to unearth and expose one’s own greed, arrogance and envy. Many of us are trying to hide such character traits in plain sight, never mind telling the world about them in a popular book.

After all, Guy is not writing about a fictional character; he is writing about himself. The humility he displays is what makes the book so effective — you can clearly follow the deliberate transformation of a cockroach (the Wall Street version of a caterpillar) into a butterfly.

This memoir is able to achieve something that many other investments books don’t (including my own): It reveals the real, practical, behavioral side of investing, not the way you read about it in behavioral finance textbooks but the raw emotions every investor experiences.

There are a lot of lessons we can learn from Guy. The first one — and, for me, the most important one — is that environment matters.

Enter Dan Ariely PhD

Dan Ariely PhD, the well-known behavioral economist, was interviewed on Bloomberg Television and asked, What can one do to lose weight? He said, Start with the environment around you. If you come to work and there is a box of doughnuts on your desk, losing weight is going to be difficult. Also, look in your fridge: All the stuff that is probably not good for your diet is staring you in the face, whereas the fruits and vegetables that are essential to healthy eating are buried in the hard-to-access bottom drawers.

The same applies to investing: We may not notice it, but the environment around us impacts our ability to make good decisions. Guy writes, “We like to think that we change our environment, but the truth is that it changes us. So we have to be extraordinarily careful to choose the right environment — to work with, and even socialize with, the right people.”

I have found that checking the prices of stocks I own throughout the day shrinks my time horizon, impacts my mood and wastes my brain cells as I try to interpret data that have very little information. I am getting better; I am already down checking prices only once a day. My goal is to do it just once every few days.

Guy is ahead of me: He checks them once a week. Recently, I put in price alerts for stocks my firm owns or follows. If a stock price changes more than 10 percent or crosses a certain important (buy or sell) point, I’ll get an e-mail alert.

Guy finds that he isn’t effective when he gets to the office because of external distractions. In his Zurich office he has a quiet room that he calls the library. It doesn’t have phones or computers, and this is where he reads, thinks and naps. Here in Denver, I have a lawn chair (bought at Costco for $50) that I take outside to sit on, put on headphones, and listen to music and read. My friend Chris goes to Starbucks or the local library in the morning for four hours before he goes to his office, and that’s where he does his reading. The key is to figure out what works for you and try not to fight your external environment.

Another lesson I have learned is that misery loves company. I was talking to Guy about his book, and he told me that people who love the book appreciate the fact that he is so honest about the emotions that consume him when he is struggling in the stock market. As investors, we often put on a brave face, but if we aren’t emotionally honest, our opinion of ourselves, our self-worth, may fluctuate with the performance of our portfolio.

Personally, I can really relate to this. When I read Guy’s book the first time (I’ve read it twice), I was going through a tough time with my portfolio. I found this book extremely therapeutic. In fact, I recommended it to a friend of mine who was going through a similar rough patch.

Another lesson:

Surround yourself with the right people. Friendships matter. I’ve been blatantly plagiarizing Guy on this for years. Guy created a conference called VALUEx Zurich, a gathering of like-minded people who get together and share investment ideas. I attended the very first one in 2010, and since then I have hosted a very similar event, VALUEx Vail, every year in June.

Guy has a latticework group of eight investors that meets every quarter and discusses the stock market, the investment process and personal issues. I’ve copied that, too. Four of us got together in Atlanta in October. We visited a few companies and debated stocks, industry trends, diets, women . . . okay, you get the point. That was our first latticework event, but I hope we’ll meet a few times a year.

Attending Guy’s conference in Zurich and organizing VALUEx Vail have resulted in enduring friendships. These conferences allowed me to create a large network of like-minded investors I talk to regularly. Every member of my latticework group I met at VALUEx Vail.

(A short side note: One of the most important things we can do as parents of teenage kids is to make sure they have good friends. That’s paramount. We as parents lose influence on our kids when they become teenagers. Their friends have a disproportionately larger impact on their choices than we do. We can influence the environment around our kids by helping them select friends.)

And then there are thank-you cards. Over the years Guy has written tens of thousands of them. He is indiscriminate about them — at one point he wrote to every employee at a boutique hotel he stayed in. All right, maybe he took it too far that time. But, writing a thank-you card to value investor Mohnish Pabrai changed his life. He attended Pabrai Investment Funds’ annual meeting in Chicago. After the meeting he sent Pabrai a thank-you note. A few months later Pabrai came to New York and invited Guy to dinner. This was the start of the Spier-Pabrai bromance. Thank-you cards work because so few people write them. They leave a lasting impression on the receiver because they say, “I like you. You are important to me.”

Mentors

The last point is, Be yourself. Having mentors is important. For many value investors, Buffett and Munger are our north stars. There are lots of things we can learn from them. But we also have to realize that we must be ourselves, because we are not them. I remember reading a long time ago that Buffett did not do spreadsheets. That impacted me for a few months — I stopped building models and creating spreadsheets. I thought, If Buffett doesn’t do it, I shouldn’t do it either. Wrong.

Buffett is a lot smarter than I am; he is able to analyze companies in his head. He is Buffett. I have found that spreadsheets work for me because they help me think. When Buffett and I look at a company philosophically, we are looking for the same things, but I need a computer to assist me, and he doesn’t.

Mohnish Pabrai owns just a handful of stocks. Guy, on the other hand, knows that he would not be able to be a rational decision maker if he had only a handful of stocks. There will be a significant overlap between Guy’s and Pabrai’s portfolios, but Guy’s will have two or three times as many stocks.

Dear ME-P Readers, I spoke with your Editor-in-Chief Dr. Dave Marcinko a few weeks ago, and as you can tell from this ME-P essay, I am a very biased book reviewer. I am not even sure this qualifies as a book review. Despite my biases, I can safely say that The Education of a Value Investor is one of the best books I’ve read in 2015. (I promise you that it is not the only book I’ve read this year.) Before you commit your time and money to this book, watch Guy’s presentation on Talks at Google.

We’ve seen the Future of Translational Medicine and it’s Disruptive

The Class

Our class talked to 2,355 customers, tested 947 hypotheses and invalidated 423 of them.  They had 1,145 engagements with instructors and mentors. (We kept track of all this data by instrumenting the teams with LaunchPad Central software.)

In a packed auditorium in Genentech Hall at UCSF, the teams summarized what they learned after 10 weeks of getting out of the building. This was our version of Demo Day – we call it “Lessons Learned” Day. Each team make two presentations:

  • 2 minutes YouTube Video: General story of what they learned from the class
  • 8 minute Lessons Learned Presentation: Very specific story about what they learned in 10 weeks about their business model

We have learned a remarkable process that allow us to be highly focused, and we have learned a tool of trade we can now repeat. This has been of tremendous value to us.

Over the last three years the National Science Foundation I-Corps has taught over 700 teams of scientists how to commercialize their technology and how to fail less, increasing their odds for commercial success.

To see if this same curriculum would work for therapeutics, diagnostics, medical devices and digital health, we taught 26 teams at UCSF a life science version of the NSF curriculum. 110 researchers and clinicians, and Principal Investigators got out of the lab and hospital, and talked to 2,355 customers. For the last 10 weeks 19 teams in therapeutics, diagnostics and medical devices from the National Institutes of Health (from four of the largest institutes; NCINHBLI, NINDS, and NCATS) have gone through the I-Corps at NIH.

87 researchers and clinicians spoke to 2,120 customers, tested 695 hypotheses and pivoted 215 times. Every team spoke to over 100 customers.

Three Big Questions
The NIH teams weren’t just teams with ideas, they were fully formed companies with CEO’s and Principal Investigators who already had received a $150,000 grant from the NIH. With that SBIR-Phase 1 funding the teams were trying to establish the technical merit, feasibility, and commercial potential of their technology. Many will apply for a Phase II grant of up to $1 million to continue their R&D efforts.

Going into the class we had three questions:

  1. Could companies who were already pursuing a business model be convinced to revisit their key commercialization hypotheses – and iterate and pivot if needed?
  2. Was getting the Principal Investigators and CEO out of the building more effective than the traditional NIH model of bringing in outside consultants to do commercialization planning?
  3. Would our style of being relentlessly direct with senior scientists, who hadn’t had their work questioned in this fashion since their PhD orals, work with the NIH teams?

I-Corps at the NIH: Evidence-based Translational Medicine 

Evidence-based Translational Medicine
We’ve learned that information from 100 customers is just at the edge of having sufficient data to validate/invalidate a company’s business model hypotheses. As for whether you can/should push scientists past their comfort zone, the evidence is clear – there is no other program that gets teams anywhere close to talking to 100 customers. The reason? For entrepreneurs to get out of the building at this speed and scale is an unnatural act. It’s hard, there are lots of other demands on their time, etc. But we push and cajole hard, (our phrase is we’re relentlessly direct,) knowing that while they might find it uncomfortable the first three days of the class, they come out thanking us.

The experience is demanding but time and again we have seen I-Corps teams transform their business assumptions. This direct interaction with potential users and customers is essential to commercialize science (whether to license the technology or launch a startup.) This process can’t be outsourced. These teams saved years and millions of dollars for themselves, the NIH and the U.S. taxpayer. Evidence is now in-hand that with I-Corps@NIH the NIH has the most effective program for commercializing science.

Lessons Learned Day
Every week of this 10 week class, teams present a summary of what they learned from their customers interviews. For the final presentation each team created a two minute video about their 10-week journey and a 8-minute PowerPoint presentation to tell us where they started, what they learned, how they learned it, and where they’re going. This “Lessons Learned” presentation is much different than a traditional demo day. It gives us a sense of the learning, velocity and trajectory of the teams, rather than a demo day showing us how smart they are at a single point in time.

BCN Biosciences
This video from team BCN Biosciences describes what the intensity, urgency, velocity and trajectory of an I-Corps team felt like. Like a startup it’s relentless.

BCN is developing a drug that increases anti-cancer effect of radiation in lung cancer (and/or reduces normal tissue damage by at least 40%). They were certain their customers were Radiation Oncologists, that MOA data was needed, that they needed to have Phase 1 trial data to license their product, and needed >$5 million and 6 years. After 10 weeks and 100 interviews, they learned that these hypotheses were wrong.

Pathetic results compared to the markets

Most investors are very good at hurting themselves financially. According to latest release of Dalbar’s Quantitative Analysis of Investor Behavior (QAIB), the average investor has a return of only 2.6% over the last ten years. That’s pathetic compared to what the markets gave. See the chart below, over the same period, the S&P 500 gave an annualized return of 7.4% and the bond market gave 4.6%.

Investor behaviors are such a big drag on investment returns that Nobel Prize winner Daniel Kahneman, an Israeli American, advised Israel’s Pension Authority to send out statements once a quarter instead of once a month. Since when Israel’s pensioners don’t get their statements, they don’t do stupid things to their accounts.

So what are those behaviors that are so costly to investment returns? Please watch this five minute long video produced by Independence Advisors.

In a nutshell, the emotional reactions (such as herding) that had helped our hunter-gatherer forebears survive so well and thus are hard-wired into our brains are literally hazardous to successful investing. In a way, the value of an advisor like myself is to separate your emotions from your money.

Rick Kahler CFPIf you keep your life savings in certificates of deposit or a savings account at your local bank, that decision may be based on a common money script: “You can’t trust the stock market.”

This belief about money can keep you from making the most of your retirement savings.

Money Scripts

I was recently interviewed by Clark Palmer for a Bankrate article about money scripts. Palmer did quite a good job of explaining money scripts, the largely unconscious beliefs about money that we all hold and that affect our behavior around money. Many of these scripts are developed in childhood. Typically they are only partially true, but sometimes we follow them rigidly even in circumstances where they are not accurate. This usually doesn’t serve us well.

Clark Palmer Speaks

In describing the problems with adhering to rigid money scripts, Palmer made this statement: “For instance, distrusting the stock market would have made a lot of sense after the economy collapsed. Since the stock market has rebounded in the past few years, it no longer makes as much sense to distrust the stock market.”

This example actually replaces one money script: “You can’t trust the stock market,” with another: “You can’t trust the stock market in poor economic times, but you can trust it when the economy is doing well.”

This second script sounds like a recipe for exactly what many investors did during the recent recession. When the market crashed in 2008, they sold stocks, taking huge losses in order to move their nest eggs out of the frightening world of the stock market and into CD’s or money market funds that seemed more trustworthy.

Missed Opportunities

Yet, by getting out of the market, they missed the opportunity to have their holdings regain value as the market recovered. Their savings earned safe but meager returns and didn’t decline further in value, but they did lose purchasing power by never regaining their losses. Now, with the market back up and appearing more stable, it seems worthy of trust again, so some of these same investors are buying stocks. The trouble is, they are now paying a premium to get back into that “trustworthy” high market.

Does this mean the first money script, “You can’t trust the stock market,” is true after all?

Not at all.

What you can’t do is trust that the market will always go up. You can’t trust that it will always go down, either. You can’t trust stocks that provided high returns over the past ten years to do the same in the upcoming decade. You can’t trust investors to make decisions about buying and selling in logical ways based on economic principles—partly because many of those decisions are based on money scripts.

Gurus of the Moment

Nor can you trust yourself or anyone else to successfully time the market, buying at just the right low point or selling at the perfect high. This is true even though there is usually a “guru of the moment” who manages to do exactly that through sheer luck.

What you can trust is that the stock market will do what it has always done. It goes up and down in response to a complex set of economic, emotional, and political factors. The way to trust the stock market is to accept the reality of what it is.

Assessment

Here, then, is my suggestion for a more accurate money script about the market: “You can trust the stock market to do what it does, which is fluctuate.”

This is why the wisest strategy for most investors is to trust the market over the long term with a well-diversified portfolio.

Did you know that Tennessee’s US senatorial candidate, Dr. George Flinn, just announced his Patient Centered Health Plan as a sustainable alternative to Obamacare?

“This country needs a strong, positive alternative,” said Flinn. “We need to unite behind a solid proposal now because the longer Obamacare is in place, the harder it will be to repeal.”

The PCHP [Patient Centered Health Plan]

The Patient Centered Health Plan advocates a quality, affordable system promoting principles such as portability, competition across state lines, and the expansion of health savings accounts (HSA).

Flinn stated that his plan aims to end the assault Obamacare has created on our liberty and free enterprise in this country.

From massive job loss, decreased quality of care, doctor shortages, layoffs in health services, and millions still uninsured, Obamacare is doing the opposite of what it was made to do.

Both parties criticize one another for different aspects of Obamacare. The only consensus is its inability to effectively function. For the betterment of the United States, party lines need to be overlooked in order to find a solution.

A MOVING PUZZLE

DEM at Univ of PittsburghLast month, I had the opportunity to visit and tour the University of Pittsburgh and the Tower of Learning.

While there, I was given this brain teaser by one of the graduate students. I was told that if you can put this moving puzzle together; you may just be able to say goodbye to Alzheimer’s Disease!

Now, this is really clever and a bit challenging. So, much as we middle-aged folks are concerned with Alzheimer’s disease, this puzzle may help dispel some fear. Give it a try. Just remember, I was told that if you can put this puzzle together … you probably do not have to fear Alzheimer’s!

The article, written by Time contributor Steven Brill, is required reading for all healthcare providers, administrators, legislators, patients — basically, everyone; especially readers of this ME-P.

The article is too comprehensive to summarize in one blurb, but Mr. Brill did a good job of describing its origins to Jon Stewart on The Daily Show.

Throughout all of the discussions during the Obamacare debate, the focus was usually on who should pay the medical bills.  Brill said, “We never asked the first question: Why are the bills so high?”

pruitt6If HHS and AHIP continue to give doctors the bum’s rush, what languages can we expect migrant providers to speak well?

The Conference

Last week, I came across a video of a health care conference held a month ago at the University of Miami. During a discussion period, a Miami spinal surgeon named Dr. Nordham warned that more Medicare pay cuts will make small, solo practices like his unsustainable.

Karen Speaks

Panelist Karen Ignagni, who is president and the CEO of America’s Health Insurance Plans (AHIP), reacted defensively in favor of continued unsustainable discounts – but with a hasty, disingenuous response: “We’re seeing out of network charges of 95 times Medicare fees.” While as if on cue, former HHS Secretary Donna Shalala, who is also president of the university, offered her cold interpretation of the small business owner’s legitimate fears: “He’s really complaining that the price is going down [according to law].”

Shalala Speaks

After also ignoring the physician’s plea, “There needs to be more transparency,” Shalala and Ignagni continue an irrelevant, buzzword-filled discussion with each other using flowing hand gestures while shutting out the doctor’s attempts to bring the conversation back on topic. Then abruptly, without giving Dr. Nordham the opportunity to say another word, Shalala slammed the door: “…. I think we’ll take the next question, thank you.” Then she threw him a bone, “It’s a very important question, though.”

The 4 minutes of unvarnished disrespect of Dr. Nordham is so transparent that one wonders whether Shalala and Ignagni were even aware that their half-baked PR game was being recorded for C-span.

Hopkins Medical SchoolA unique speech was delivered by neuro-surgeon Benjamin Carson MD on February 7, 2013 at the National Prayer Breakfast in President Barack Obama’s presence.

Who is Ben Carson MD?

Benjamin Solomon “Ben” Carson, Sr. (born September 18, 1951) is an American neurosurgeon and the Director of Pediatric Neurosurgery at Johns Hopkins Hospital. He was awarded the Presidential Medal of Freedom, the highest civilian award in the United States, by President George W. Bush, in 2008.

The Breakfast

During the breakfast, Carson suggested that political correctness is a “dangerous” threat to free speech and encouraged Americans to share their views without hesitation. Carson also included his ideas on the national debt, deficits, taxation and health care; he explains his personal position on each matter.

Here is a teaser quote:

I don’t like to bring up problems without coming up with solutions… What about our taxation system? It is so complex, there is no one who can possibly comply with every jot and tittle. That doesn’t make any sense.

What we need to do is come up with something that’s simple. The inherently fair principle is proportionality: you make 10 billion dollars, you put in a billion. You make 10 dollars, you put in one. Of course, you have to get rid of the loopholes.

Some people say, ‘That’s not fair! It’s doesn’t hurt the guy who made 10 billion dollars.’ Where does it say you have to hurt that guy? He just put a billion dollars into the pot!

My Connectinon to Ben Carson

Ok, I really don’t have any connection to Dr. Carson despite the seven degrees of separation philosophy. But, I did grow up in Baltimore Maryland and played stickball in the parking lot of the famed Johns Hopkins University  Hospital. I was even seen in the ER for a minor injury as a kid.

But, I was not accepted into medical school there, and could not attend Johns Hopkins University up on North Charles Street for my undergraduate career, because of the expense.

The Video

Nevertheless, this video is worth watching. It is 26 minutes in length and it is interesting to watch the president grimace as he gives a complete opposite solution to every problem the country faces.

Ben’s Proposals

I especially liked Ben’s thoughts on the following topics:

  • Replacing the IRS with tithing for all income levels. No need to hurt the successful among us with a graduated tax system.
  • Giving all Americans a Health Savings Account [HSA] at birth. This will not only give them some financial skin-in-the game, but makes them educated stewards of their healthcare needs, treatments and expenses. And, the savings portion would be transferrable to a next generation beneficiary for estate-like continuity.
  • Giving everyone a personal electronic health record [pEHR] at birth.
  • Reforming the welfare state so it does not become a way of life
  • Morality and the PC mania.

Assessment

Ben is one smart pediatric brain surgeon. I would consider voting for him in a heart-beat. But, as a surgeon, I am like him, a doer who wants to solve a problem.

Unfortunately, Washington politicians are often talkers who place self-interest above all. Problem solving often takes a back-seat to pleasing constituents.

Bank Offers a Zero Down Payment Physician Mortgage Loan in Kansas and Missouri

Doctors mortgage programs are offered as a benefit in many hospitals. Banks use these physician mortgage loans as an entry point to gain checking, savings, investment, and Home Equity Line of Credit accounts.

It’s important for physicians to realize that a Big banks objective is assets under management and not necessarily the best mortgage for them.

Many banks offered  special  zero down doctor loans and below market mortgages for physicians.  With the upheaval in the mortgage and secondary market requirements (the people who bought those special physician loans), most of the doctors mortgage programs advantages went away, or became no different than what is available to every other borrower.

For this reason, it is prudent for physicians to be aware of the changes in doctors loan programs and seek expert independent 2nd Opinions consultations.
