It is well known that the flash crash of 2008-09, and the continuing hangover from the recent recession, is a drag on the income of many financial advisors; especially those who derive their salary from some percentage of assets under management. But, the markets have bounced back in the last decade to near all time highs, in 2019!

Business Models and Definitional Haze  

Regardless of definitional haze, it seems that commissioned salesman, stock-brokers, financial planners, insurance agents and financial advisors of all stripes – and business models –  have been economically hurt as the nation’s unemployment rate hovers at just under ten percent.

Broker-Dealers [BDs] and Registered Investment Advisors [RIA] are not immune; as this voting poll was first launched in 2011.

A survey in 2011 suggested that more than 40% of the country’s doctors are doing less pro-bono work due to managed care, and the resulting decrease in personal income.  Today, some pundits wonder if the exacerbated cause was the ACA?

To combat this unintended economic phenomenon today, the organization Volunteers in Healthcare – now with the American Academy of Family Physicians – offers a free information patient record system to track the medical care given to the uninsured. The system allows physicians to track and store information on patients, visits, providers, clinics, referrals and more.  It is guide-driven with sample reports that can be reconstituted to provide summary statistics on patients and providers.

A 2015  Medscape Compensation Report sheds light on physicians’ earning potential.

Here are some key findings from a survey of 20,000 physicians in 26 specialties:

  • Orthopedists ($421,000) and cardiologists ($376,000) are still the top earners among physicians.
  • Physicians in private practice earn significantly more ($329,000 for specialists) than do employed physicians ($258,000 for specialists), despite the trend toward employment.
  • Male physicians earn more ($284,000) than their female counterparts ($215,000).
  • North Dakota and Alaska ($330,000) are the top-paying states for physicians, while Rhode Island ($217,000) and Maryland ($237,000) are the lowest-paying.
  • 9% of physicians have concierge or cash-only practices, the same percentage as last year, while ACO participation continues to grow.

A recent survey of 3,729 family care physicians found that 40.3% considered leaving their primary care practices, with 16.5% reporting that 2010 was the first year they had considered such a career change.

M3 Global Research

“The poll results are not surprising given the rising financial pressures for family practices,” said Craig Overpeck, chief operating officer of M3 Global Research.

“Only 15.6% hold out hope of 2011 being a better than average year for their personal income, with 17.7% forecasting 2011 to be one of the worst earning years of their career. The survey also reported only three out of five physicians enjoying better job satisfaction than they anticipated on their first day in medical school.”

Last week marked the first anniversary of the Affordable Care Act (ACA) being signed into law by President Obama, and one year into the new era of health care reform it’s clear that Americans remain divided in their views on the ACA.

Depending on the source, polls show the public remains confused about many aspects of the law, with mixed support for several provisions and strong opposition to the individual mandate and other parts of the ACA.

Legal Challenges

With several lawsuits challenging the constitutionality of the ACA, governors and state legislators vowing to refuse funding to implement certain ACA programs, and Congress poised to revise or repeal some or all of the law, opponents of the ACA are hopeful that they will have the chance to go back to the drawing board to craft reform legislation more to their liking.

Supporters

Meanwhile, supporters are pointing to widespread public approval of many of the insurance reforms in the law and claiming that once the health exchanges and other major components of the ACA take effect, public support will continue to grow.

The ME-P Wants to Know:

Today, when patients communicate through instant messaging, Twitter, Facebook, and other Web 2.0 electronic mediums, they might feel that health providers are already more like the virtual “Doctor” than the all-too-human “Bones.”

The Contemporary Practice Milieu

Before long, according to one technology expert, 20% – 50% of all doctor-patient communication will be virtual. But we suggest you pause before rocketing ahead into this brave new future that advocates call Health 2.0—the application of social media tools to the health care environment.

Electronic technology in all of its forms has obviously had a profound impact on medicine. We focus here on just one of its most notable effects: the changing doctor-patient relationship. We believe Health 2.0 has the potential to deepen this relationship—or not. It depends on how you use it.

Our Guidance

There are an almost overwhelming number of social media tools for managing the doctor-patient relationship. How do you choose the right ones? We offer some guidance in this essay by focusing on three issues:

  • What matters most in the doctor-patient relationship?
  • What counts as a good relationship?
  • How should you use social media tools to build a relationship?

We have found that there is no one best way to use Health 2.0 technology. But, there is just one rule. As the novelist E.M. Forster said, “Only connect.”

The Survey

And so, we ask you to opine:

  • Has your doctor-patient relationship changed in recent years with the rise of the Internet search engines like “Dr. Google and Dr. Oogle” [for dentists] and the push to empower patients to take a greater role in their own care via HD-HCPs, private or direct payment models, etc?
  • Are patients more demanding of your time and attention than in the past? Do they understand the economic pressures that affect your practice? Do they care, or should they even care?
  • How do you handle noncompliant or uncooperative patients? What strategies work best or least? Is this issue underappreciated by the people pushing to base a greater portion of reimbursement on quality measures and outcomes?
  • How much time each week do you spend on paperwork, phone calls to payers, insurance companies, and other administrative tasks? How much has this increased in the last few years? Have you reached your breaking point, yet?

A NPR / Kaiser / Harvard School of Public Health patient opinion poll of more than a year ago [Aril 2009], demonstrated that for the most part, patients believed that just spending money on eMR’s was not going to improve their health or bring down health care costs.

In fact, the most important part, it seems, is their relationship with their doctor [ie, trust].

“Primary care physicians and other specialists will continue to be the target of acquisitions by larger health and hospital systems. Physicians will begin to transition to a more consumer-based patient orientation model (i.e. concierge medicine, cash-for-services, retail) in order to replace lower reimbursements, or opt out of third-party reimbursement models entirely. Or alternatively, these physicians will shift to become an employee of a larger system.”

The Social Transformation of American Medicine [The rise of a sovereign profession and the making of a vast industry].

This classic book was written by Paul Starr in 1984. I first read it while in business school back in 1994-96. Here is an excerpt from pages 227 and 232. It is even more relevant for the healthcare industrial complex today.

Quoting Kenneth Arrow PhD

The structural features Arrow[*] discusses have a history. He writes that when the market fails, “society” will make adjustments. […] But, modern day economists like Austin Frakt PhD and others, have to ask: For whom did the market fail, and how did “society” make these adjustments?

Of Failed Markets

The competitive market was failing no one more than the medical profession, and it was the profession that organized to change it. […]. By the 1920s, the medical profession had successfully resolved the most difficult problems confronting it as late as 1900. It had […] won stronger licensing laws; turned hospitals, drug manufacturers and public health from threats to its position into bulwarks of support; and checked the entry into health services of corporations and mutual societies. It has succeeded in controlling the development of technology, organizational forms, and the division of labor. In short, it had helped shape the medical system so that its structure supported professional sovereignty instead of undermining it.

Master over Diseases

Over the next few decades, the advent of antibiotics and other advances gave physicians increased mastery of disease and confirmed confidence in their judgment and skill. The chief threat to the sovereignty of the profession was the result of this success. So valuable did medical care appear that to withhold it seemed deeply unjust. Yet as the felt need for medical care rose, so did its cost, beyond what many families could afford. Some agency to spread the cost was unavoidable. It would have to be a third party, and yet this was exactly what physicians feared. The struggle of the profession to maintain its autonomy then became a campaign of resistance not only to programs of reform but also to the very expectations and hopes that the progress of medicine was constantly arousing. To continue to escape the corporation and the state meant preserving a system that was at war with itself.

In healthcare, for example, the use of patient website portals is a hotly debated topic. These are [should be]  sophisticated HIPPA compliant and secure Web sites offered by medical practices to help engage patients electronically, with the promise of better service and care for patients — and less hassle for the medical practice, doctors and nurses. Often clinical, insurance and financial data gathering and scheduling functions are included, along with separate patient log-in, e-prescribing, and laboratory result features, etc. The promise of eMRs only increases the sophistication of these burgeoning sites.

Use Still in Infancy

But, according to www.MedicalBusinessAdvisors.com unscientific sampling of our clients and technically sophisticated practices [skewed cohort], physicians note that the uptake of portal use by patients outside of tech savvy urban centers is still small, although use by senior citizens is rapidly increasingly. And, tech savvy youngsters are typically not in need of healthcare.

The Survey Question

So, this raises the question, unanswered by other professionally focused websites like Physicians Practice, should you make the patient portal leap?

Definitions

Before we answer that question, let’s provide a bit more historical detail on this technology. In contrast to a traditional [first generation – health 1.0] practice Web site, which provides smiling pictures of the physicians, directions, hours of operation, policies, and maybe a smattering of educational materials, a patient portal is designed for active interaction between patient and practice [second generation – health 2.0].

Example:

As an example, a patient portal typically provides secure e-mail, allowing the patient to make a quick query of the physician (and presumably receive a reasonably quick response) without the delay and inconvenience of attempting to catch the physician on the phone between visits or after hours. Patient portals can also be used for open-source scheduling, allowing patients to make requests for particular times and days.

Assessment

Finally, the newest and most sophisticated patient e-MR engaged portals will allow patients to take a peek inside their patient record, giving them online (and secure) access to their medication list, recent labs, and other data that might be useful in self management, or if the patient is seeing another provider, etc.

The success of this study depends on the survey being distributed to the appropriate people to be completed. To show CHG’s appreciation you will receive a complimentary copy of the results including the trended data from the 2005 and 2008 studies.

The Physician Sales and Service Survey  

Physicians still direct the vast majority of inpatient healthcare in the marketplace – as many as 80% of patients enter the doors of hospitals and facilities at the direction of physicians. Notwithstanding he movement toward a consumer-driven market – many hospitals, health systems and large specialty practices have turned to physician sales or referral development programs to grow their business. Unfortunately, motivating physicians to change referral patterns is a daunting task under the best of circumstances and the lack of industry “best practices” complicates the situation even further.

Third Benchmarking Study

September 2010 Corporate Health Group (CHG) launched their third benchmarking study of Physician Sales and Service. The National survey will obtain new trends and craft comparisons and a gap analysis of data captured 2005, 2008 and now 2010. The survey (a mix of open and closed ended questions) is available online and respondents will get a free Executive Summary. The results will provide detailed management and benchmarking data for physician sales managers and healthcare executives to benefit their programs for future success!

According to behavioral economists and financial advisors like Brian Knabe MD CFP™ CMP™ people feel the pain of loss twice as much as they derive pleasure from an equal gain. Studies show that humans process investment losses using the same part of the brain that responds to mortal danger.

For example, a 10% loss in the market causes twice the emotion as a 10% gain would elicit, and the short time period involved in the “flash crash” of 2008-09 compounded this effect.

According to a new NBC News/Wall Street Journal poll, the public has soured on President Barack H. Obama’s health care reform plan.

In fact, former Governor and Democratic National Committee Chairman Howard Dean MD told Vermont National Public Radio:

“This is essentially the collapse of health care reform in the United States Senate. And, honestly, the best thing to do right now is kill the Senate bill and go back to the House … You have the vast majority of Americans want the choices, they want real choices. They don’t have them in this bill. This is not health care reform and it’s not close to health care reform.”

At least one iconoclastic physician, ME-P thought-leader Al Borges MD, has asked that all doctors unite and participate in this year’s Medicare “HIT 1115 Project”; now in-process.

The November 15, 2009 Project

November 15, 2009 began the 6-week time period during which all medical providers can switch Medicare participation. If all physicians become “non-participating” or simply “opt-out,” then lawmakers and their lobbyists may take notice that doctors are fed-up with government intrusion into physician affairs!

Recovery Asset Contractor Survey Poll

RAC RESULTS TO-DATE [Beta]

According to the Centers for Medicare and Medicaid Services [CMS], RACs collected about $1-B in improper payments during their recent beta testing period. Of these payments; 96% were over-payments, 4% were under-payments; and 77% of providers failed to appeal, 7% appealed successfully and 15% appealed unsuccessfully.

Going forward there will be a three year “look-back period”, and a 10% contingency payment level for the four regional RACs currently in the program:

  1. Connolly Consulting
  2. PRG-Schultz
  3. HealthDataInsights
  4. Diversified Collections Services

By 2010, the RAC program is scheduled to launch in all 50 states. And so, please cast your vote in our exlcusive ME-P RAC program survey poll.

As you know, this Friday marks the close of the first reporting period for Recovery Act funds. Any grants or loans awarded between February 17th (the signing of the bill) and September 30th 2009 will be reported in the survey.

I am eager to see the results because I want to know what effect the economic Stimulus Bill has had on eMR adoption rates. Are more doctors buying eMRs as a result of incentives? Or, has the bill simply reinvigorated research?

Our Recent Experience with CFP® Mark Utility

Certification Falling from Grace – Deserved or Not?

The Premise

In the summer [2008], we sent a random email blast to the first 200 Certified Financial Planners® on our list-serve. These were folks who had previously contacted us, and/or purchased our textbooks, handbooks, tools and/or dictionaries that assist accountants, financial advisors, attorneys, medical management consultants and all those working to assist physicians and medical professionals on business and economics matters.

The “Straw-Poll” Query

Our email blast asked the simple question:

“Did you ever voluntarily resign your license to use the CFP® mark?”

First Round Results

We received four positive responses [2%]. We then followed up to learn that 2 of the 4 were CPAs, one was a CFA and another was an MBA. Now, what do these results signify – probably nothing – or maybe an emerging trend?

So, last summer [2009], after the continuing Wall Street collapse, and the Somnath Basu PhD article on “CFP Trust” in Financial Advisor magazine and this blog, we sent out a follow-up email to the exact same 200 Certified Financial Planners® as before; but carved-out and replaced the 4 CFPs who had resigned the mark, with 4 others.

This time we asked the question:

“Have you recently considered allowing your CFP mark to lapse; or resigning it?”

Second Round Results

This time we received exactly eight positive replies [4%] or double the number from the first round. One CFP® said:

“I am rethinking my entire business and marketing philosophy. This includes separation from any taint left over from recent industry scandals – and yes – even including my CFP® mark”

Assessment

This little experiment was not statistically significant by any means. And, again it probably is indicative of nothing. Yet, these types of questions must be boldly asked today; even if they were not even timidly asked yesterday.

Nevertheless, cited plausible reasons for the increased negative CFP® mark response may be:

 

  • CFP BoS lacks modernity and membership alliance. 
  • SEC mismanagement.
  • NASD/FINRA impotence.
  • Wall Street greed.
  • Lack of true fiduciary accountability.
  • Client anger and public distrust.
  • Advisor frustration at lost income.
  • College for Financial Planning and American College credibility.  
  • ME-P operations in the medical niche advisory space.
  • CFP® mark and related industry certification taint.
  • Alternative degrees and available designations.
  • Rise of RIAs and the fiduciary CMPmark for healthcare specificity.
  • Resigning [doing] and considering [thinking] are not equivalent;
  • etc, etc. 

It is interesting to note that no CFP® resigned their mark who did not hold either another graduate degree [MBA, MSFS, MA, MS, PhD], or more rigorous industry [CFA and CPA] certification.

Assessment 

So, is CFP mark allegiance just a union-like mentality of “united we stand – divided we fall”, by those with little to no gravitation pull of their own – or something else; ie., industry group think? You decide; and do tell us what you think.

Our New ME-P Rating System

Drilling Down on our Rating System –  Eyeing your Rankings

The direct rating of posts is now available on the ME-P. This enables you to rate each post individually using the traditional five star system [one star = very poor post – five stars = excellent post]. To get started, just look for the five red-star ratings system at the bottom of each post. Then, Read * Submit * Share * Rank * Publish * Prosper.

Assessment

Until now, ME-P readers were only able to indirectly rate each post. These ratings appear on the left home page side-bar, under the heading Engagement Rankings, and are updated hourly. It was a pretty decent system.

But, with room for only 15 rankings, those posts thereafter were never prioritized or appreciated. So now, these worthy posts – and contributing authors – can still be appreciated by their fans. You can even rate this new 5-star rating system feature; below!

About Marquette, Michigan

As our ME-P readers are aware, Marquette Michigan has a population of 20,714, and is the UP’s largest community. In addition to being a population center, it serves as the regional center for education, health care, and outdoor recreation. This regional draw is particularly evident due to Northern Michigan University and Marquette General Hospital [MGH]. Naturally, during my recent tour there, I was able to visit both small and large medical practices, clinics and hospitals. Everywhere, the topic of conversation was the Obama Administration’s vision of domestic healthcare reform.
