On Being Un-Employed or Under-Employed

When one [young medical professional] works at a job for which he or she is overqualified, it is called underemployment. Being employed or even being underemployed beats being unemployed, any day. Since 2008, we have officially lost about 10 million jobs. These are often people who have families and children to support. As a nation, our first priority is to try and figure out how we can get all (or most) of these people back to being productively or gainfully employed before we can start considering how and when our economy will recover. The actions that the government should consider the most are also those that resolve this situation the soonest. Why so?

The Facts

Consider the fact that we have lost what these 10 million people produced. The only reason we are not seeing price increases because of this shortage of production is simply because these same people and others are not spending or willing to spend at this point, and rightfully so. The upward spike in the personal savings rate since 2008 (from about 0% in 2006 to 6.4% today) is a vocal if implicit testimony to how scared most people are today about their economic well-being. These 10 million people have also lost their incomes and therefore their own abilities to consume as before. Their standards of living have declined significantly. The government does not receive the taxes from their income either and hence cannot plow it back into the economy. Everyone is losing here, all of us, because of this unemployment. That is why it is our priority, us common people, to figure out how to increase employment for those who need it badly to get out of their current dire straits and help our entire economy. Now is not the time to point fingers, expect government created economic miracles or to play politics for votes.

A Few Possibilities

Here are a few possibilities to consider. If you are unemployed and there is no suitable jobs for you, you have to find something (and if necessary compete for it) that you can do. It could be jobs to maintain people’s homes and yards, or to clean pools and wash cars, or to drive buses, or to become a janitor, or to become a city bike courier. It could be whatever you can do best other than what you were trained for if jobs are that rare in your field.

Current Conditions

Remember, under current conditions, being underemployed is always preferable to being unemployed. No task should be (and is not) too menial, too mindless. Look around: How much does it cost every time you need a plumber or an electrician or another trades person at home? Why so? Compete here for these jobs. That helps everyone by adding value to the economy. Production at home (fixing whatever you can) and outside (fixing stuff for others), vegetable gardening if you have a little land, sewing, raising chicken for eggs, whatever value you can produce, do so!

Going Abroad

Going abroad and trying your luck outside is another alternative. More and more Americans are seeking their new careers or fortunes in foreign land. Wherever, there are opportunities. In Brazil, Russia, India and China; of course! There are others like Canada, Australia or Vietnam. These economies have done much better than ours of late. Our skills could find greater value there – and why not? Foreigners have immigrated here through the centuries for similar opportunities as well. We are more experienced than all others on immigration – let’s use this experience to our advantage. Our emigration should go hand in hand with reduced immigration domestically as well. Nothing will deter immigrants (legal or illegal) from moving than the lack of opportunities; just like the reverse is true for emigrants. If you know anyone overseas or have done anyone outside any favors, call them in now. Now is the time to ask them to help you find a job in their country. Absolutely no shame here either.

Of Small Businesses [Private Medical Practices]

Another fact that we often forget – small businesses like private medical practices in our economy – are the largest employers of people. This is your time to support your local doctor, family practitioner, internist, or ‘Mom & Pop’ establishments whenever you can.

For example, sell any veggies you can grow in your local farmers’ markets and buy your needs from there as well. Now is the time for home-grown or small-scale production to organically spread and take a grassroots hold on employment, just like it has done many times before.

Assessment

If you are so lucky to find a job, start with saving whatever you can. Your savings (investments) will not only generate jobs and help everyone else; it’ll help rebuild your assets. Only consumption spending impoverishes our future, including those of our children.

Conclusion

This time around, we need to work together to get out of this economic morass and stay out for a long time to come. The way out is by generating new jobs, vocations, opportunities. We need to accomplish this task any which way we can – and do so in a concerted effort. Only then can we begin to relax about our future.

How Should Financial Advisors Prospect and Make Sales Calls to Doctors?

Improving Contact, Communication and Closure

It doesn’t matter if you’re cold-calling a doctor’s home or office, making a return visit or have a scheduled office appointment – if you call on the doctor prospect without knowing the MD, person or background of the physician’s office, it’s all an attempt at futility (or worse, humiliation). So, don’t create the habit of an unprepared financial advisor or salesperson.

The Opening

A good financial advisor or salesperson has a plan for directing the conversation and positioning the desired outcome. The most important piece of the conversation to script is the opening questions as they can make or break the tone of your entire meeting and, ultimately, impact the outcome.

The Steps

As your call or meeting progresses, consider these steps:

  • Focus questions on the doctor, their practice, business or personal needs.
  • Use questions to first establish a dialogue to share and learn what is most important to them.
  • Value the information they share with you and look for ways to respond that will provide a solution.
  • Demonstrate your sincerity in learning about the physician.
  • Probe, probe and probe again.
  • Work through the layers of information to understand their emotional and task reasons for the purchase.
  • Use questions to learn their frame of reference.
  • Questioning will reveal their ideas for potential solutions.
  • Learn the value and priority of finding a solution and the doctor’s sense of time frame, finances and value-added options.

Summarize the Questions

  • This will ensure you capture input correctly and “take their pulse” before moving forward to the next step.
  • Develop your solutions.
  • Address medical practice, business and personal reasons for buying.
  • Make sure your solution is benefit-oriented.
  • Don’t “Feature Dump.” Anyone can match your products or service features – it’s you and the benefits that set you apart and cannot be [so] easily replicated.

Congress Adjourns with Unfinished Tax Business

Government Operations and Taxes Dominate Agenda

On September 29th, the House of Representatives passed H.R. 3081 and provided for continuing federal government operations until December 3, 2010. President Barack Obama signed the bill on September 30th and government operations will continue until December using last year’s budget.

“Lame-Duck” Session

Following the November election, Congress will return and pass budget bills for fiscal year 2011. The short “lame-duck” session will be very busy because Congress faces major decisions on the budget bills and also has not completed work on 2011 taxes.

Tax Cut Support

A group of 31 House Democrats sent a letter to Speaker Nancy Pelosi (D-CA) and supported a 2011 extension for all of the 2001/2003 tax cuts. If there were a deadlock in Congress in November, then there would be a large tax increase for all Americans on January 1, 2011. However, the White House and Speaker Pelosi have supported extending the “middle class” tax cuts, but requiring single persons with incomes over $200,000 (married couples over $250,000) to pay taxes at a higher rate.

House Minority Whip Eric Cantor (R-VA) expressed concern that there was not a vote on extension of the tax cuts. He indicated that with the combination of support by most Republicans and 31 Democratic House members, the tax cuts could be extended for 2011.

The November Agenda

In the November session of Congress, there is a need to address income taxes, capital gains tax and estate taxes:

1. Income taxes – The White House and Speaker Pelosi propose extending the middle class tax cuts and allowing the top two income tax brackets to increase to 36% and 39.6%. Most Republicans and the 31 Democratic House members would extend all of the tax cuts.

2. Capital Gains Tax – The White House and Speaker Pelosi would increase the tax from 15% to 20%. Most Republican House members and many moderate Democratic members would retain the 15% rate for 2011.

3. Estate Tax – If there is a deadlock in Congress, the estate tax exemption returns to $1 million and the tax rate will be 55%. The White House and House Democratic leaders have generally supported an exemption of $3.5 million with a rate of 45%. The last proposed compromise by Sen. Jon Kyl (R-AZ) and Sen. Blanche Lincoln (D-AR) was for an exemption of $5 million with a rate of 35%.

In this edition of Plan Management Navigator, we summarize the cost benchmarks of health plans focused on Medicaid.

Administrative Expenses

Core administrative expenses for selected Medicaid focused health plans was 8.4% of premiums in 2009, both overall and in the Medicaid HMO product itself. Comprehensive products had total PMPM costs of $24.90 and the Medicaid HMO product had total PMPM costs of $22.77.

The median values for core administrative expenses for the Medicaid HMO products offered by these health plans were 7.5% of premium and $21.30 PMPM.

Core Costs 

Core costs declined by 5.2% PMPM on an as-reported basis compared with a decline of 1.4% last year. Holding product mixes constant, costs declined by 5.8% PMPM compared with a PMPM decline of 1.4% PMPM. Central to the decline was a 13% PMPM decline in Account and Membership Administration costs.

The peer group in this analysis consisted of ten Medicaid plans, which together served 3.9 million members. Of these members, 2.8 million were Medicaid HMO, and this product comprised on average 70.8% of the total.

HealthCare Reform 

Health care reform is expected to have a dramatic effect on plans serving Medicaid-eligible members as Medicaid eligibility is expected to increase by 16 million members. But, this takes place in a weak economy which contributes to membership growth while federal and state tax revenues are under pressure. Medicaid health plans are heavily committed to the management of administrative expenses.

The summit, organized by the Foundation for the National Institutes of Health in partnership with the National Institutes of Health (NIH) and the mHealth Alliance, will be held November 8-10 in Washington, DC.

Federal EHR Incentive Program Alert

On July 28, 2010, final regulations implementing electronic health record (“EHR”) incentive programs under the HITECH Act were published. These new regulations establish the requirements for hospitals, physicians and certain other health care providers to receive federal incentive funds for their purchase, implementation and use of EHR systems. These regulations supersede the interim and proposed rules that were published earlier this year regarding the EHR incentive programs, and in many cases, the new rules lessen some of the participation requirements. However, there still remain substantial hurdles for providers to jump in order to qualify for federal funds.

What EHR Incentive Programs Are Available?

Under the final regulations adopted by the Centers for Medicare & Medicaid Services (“CMS”), EHR incentive funds are available under the Medicare and Medicaid programs.

An eligible professional may receive an annual payment equal to 75% of estimated allowable charges under the Medicare physician fee schedule for covered professional services provided by the professional in a given year. The total of all payments to a given professional is capped at $44,000 under the Medicare program.

Eligible professionals with a sufficient Medicaid patient population can receive up to $63,750 in payments under their State’s Medicaid program. Eligible professionals must choose to participate in either the Medicare or Medicaid incentive program and cannot receive funds in a given year under both programs. However, an eligible professional may make a one-time switch from one program to another.

Unlike eligible professionals, eligible hospitals may receive incentive funds under the Medicare and Medicaid programs in the same year as long as they meet the requirements for participation in both programs. The regulations describe the formulas under which a hospital’s incentive funds are calculated based on their annual number of patient discharges and other factors.

There are also special incentive programs for Critical Access Hospitals and participants in the Medicare Advantage program.

eHRs

What Professionals Are Eligible to Receive EHR Incentive Funds?

Under the Medicare program, eligible professionals include physicians, osteopaths, doctors of dental surgery or dental medicine, podiatrists, optometrists and chiropractors. In order to participate in the program, at least 50% of an eligible professional’s patient encounters must occur in a practice/location equipped with a certified EHR system.

Under the Medicaid program, physicians, dentists, nurse practitioners, certified nurse midwives and physicians assistants may participate. However, physicians assistants are only eligible to receive EHR funds if they practice in a Federally Qualified Health Center or Rural Health Clinic led by a physician assistant.

Hospital-based professionals are not eligible to receive EHR incentive funds. A “hospital-based” professional is one who furnishes 90% or more of his/her Medicare covered professional services in a hospital setting.

What Are the Requirements for Receiving EHR Incentive Funds?

Eligible hospitals or professionals must demonstrate that they are meaningful users of certified EHR technology. For participants in the Medicaid incentive program, States may adopt requirements for participation in addition to the CMS requirements.

Pursuant to regulations adopted by the Office of the National Coordinator for Health Technology (“ONC”), an EHR system must be certified by an independent body approved to review EHR systems and certify that the EHRs satisfy certain functional requirements. The ONC regulations describe detailed functional requirements for EHR systems which EHR vendors will need to demonstrate in order to obtain the certification of their systems.

In order to qualify for incentive funds, eligible hospitals and professionals must not only acquire certified EHR systems but also demonstrate to CMS that they are meaningful uses of the EHRs. The requirements for meaningful use are detailed in the regulations (including a helpful chart which summarizes the requirements). The regulations include thresholds for eligible hospitals and professionals to meet in order to demonstrate meaningful use of EHR functionality. For example, eligible professionals are initially required to demonstrate that more than 40% of all permissible prescriptions written by the professionals are transmitted electronically using the EHR. Other Stage 1 requirements concern the use of clinical order entry, active medication and allergy lists and other clinical application functions. CMS refers to the initial meaningful use requirements as Stage 1 requirements, and it intends to strengthen its meaningful use standards in phases over time by issuing Stage 2 and Stage 3 requirements.

When Can Eligible Hospitals and Professionals Receive Incentive Payments?

Eligible professionals who are meaningful users of EHRs are eligible to receive Medicare payments in calendar years 2011 through 2015 according to payment schedules specified by the regulations. In order to receive Medicare payments, eligible professionals will be required to certify to CMS that they have met all of the requirements for incentive payments under the Medicare program on an annual basis. Payments will be made annually once the required certification are made, and payment amounts will depend on the first year in which an eligible professional becomes eligible for program participation.

Eligible Hospitals who are meaningful users will be eligible to receive Medicare payments in federal fiscal years 2011 through 2016, depending on when they first participate in the program.

For both eligible professionals and eligible hospitals, payments will be made on a rolling basis by a CMS contractor. A single annual payment will be made once an eligible hospital or professional has demonstrated meaningful use to CMS for a given year.

State Medicaid programs will establish their own payment mechanisms, and we expect that each State will issue its own implementing regulations regarding its EHR incentive program.

Can Eligible Professionals Reassign their Incentive Payments to their Employers?

Yes, eligible professionals are permitted to reassign their incentive payments to their employers or to entities with which they have a contractual arrangement requiring the entity to bill for their professional services. Therefore, employers and entities that have such contracts with eligible professionals must make sure that the reassignment provisions cover the reassignment of EHR incentive payments. We can provide clients with a simple reassignment form for this purpose, upon request.

Are There Penalties for Hospitals and Professionals Who Fail to Become Meaningful EHR Users?

Yes. Beginning in 2015, there will be Medicare payment reductions for eligible hospitals and professionals who are not meaningful EHR users. Eligible professionals who are not meaningful EHR users will receive a 1% reduction in Medicare physician fee schedule payments in 2015, a 2% reduction in 2016 and a 3% reduction in 2017 and annually thereafter. There is a mechanism for hardship exceptions in limited circumstances.

Assessment

Hospitals who are not meaningful EHR users beginning in federal fiscal year 2015 will be subject to reductions in their market basket adjustments until they can demonstrate meaningful EHR use.

‘Medicare Tax’ Now to be Called ‘Unearned Income Medicare Contribution’

The payroll tax increase that was formerly known as “Medicare tax” in both President Obama’s health care reform reconciliation proposal and the original House reconciliation bill is NOT A TAX. Repeat: NOT A TAX.

Sure, individuals with earnings over $200,000 and couples with earnings over $250,000 will have to fork over 3.8 percent of their capital gains (which were not formerly subject to Medicare taxes) to Medicare, in addition to .9 percent more of their earned income.

But, among the 15 pages of changes to the Reconciliation Act [1] included in the manager’s amendment [2] released over the weekend was a wee name change: references to ‘Medicare tax’ were deleted, and replaced with the much gentler, and voluntary-sounding, ‘unearned income Medicare contribution.’

We called Speaker Nancy Pelosi’s office for more insight, and will pass along anything we hear back.

Bethesda, MD (July 19, 2010) — The Board of Directors of the Foundation for the National Institutes of Health (FNIH), after an extensive search, has named Dr. Scott Campbell as its new executive director and chief executive officer.

“We are delighted Dr. Campbell will be at the helm of the foundation and, given his experience and background in biomedical, programmatic and donor development, are confident in his ability to take the organization to the next level,” noted FNIH Chairman Dr. Charles A. Sanders.

Moments ago, the Senate reached a deal and passed a short-term six month physician pay cut, blocking the 21 percent reimbursement cut scheduled to go into effect today.

This update comes after failing on Thursday to pass the larger H.R. 4213, which not only would have provided the physician payment relief, but also included provisions to extend tax and jobless benefits.

We quantify the relationships of customer services costs to simple algebraic and other operational metrics within the Customer Services function. This analysis draws on data from the 2009 Sherlock Expense Evaluation Report.

And, in June, AHRQ plans to award a follow-on project for the development, testing and dissemination of an easy-to-use, objective and evidence-based toolkit to evaluate critical aspects of eHR systems’ usability, accessibility and information design.

In addition, the National Institute of Standards and Technology is currently seeking applications for development of an eHR usability evaluation framework, and the Office of the National Coordinator for Health Information Technology, AHRQ and other agencies are collaborating on a joint conference in July to promote collaboration in HIT usability.

The Paul Wellstone and Pete Domenici Mental Health Parity and Addiction Equity Act was enacted into law on October 8, 2008.  This act will end health insurance benefits inequity between mental health/substance use disorders and medical/surgical benefits for group health plans with more than 50 employees. The regulations that guide implementation of this law will be effective for health plan years beginning on or after July 1, 2010.  These regulations have significant implications for providers of mental health services, payers, employers, and consumers.

David Wellstone (Dave) will be giving the keynote address.  Mr. Wellstone is the son of the late Senator Paul Wellstone (D-MN, 1990-2002).  Along with his brother Mark, Dave is the Co-Founder of Wellstone Action, a national center for training and leadership development for the progressive movement.  Dave is the Co-Chair of the Wellstone Action Advisory Board and contributes to advocacy efforts on behalf of mental health and domestic violence.

Also speaking is Carol McDaid, co-founder and Principal of Capitol Decisions Inc.  Ms. McDaid led the Parity NOW Coalition behind passage of the 2008 “Paul Wellstone and Pete Domenici Mental Health Parity and Addiction Equity Act.”  Ms. McDaid has nearly 25 years of federal legislative experience in Washington, providing her clients with legislative and public affairs consulting on issues that span the breadth of health care, including Medicare, Medicaid, private sector reimbursement issues, and comparative effectiveness research.

You may have seen that the government recently released its annual report on the quality of health care Americans receive. While there have been some improvements, the news was not positive: hospitals still have work to do to put an end to the ongoing, but very solvable, problem of patients acquiring life-threatening infections in hospitals.

Should Banks Pay a TARP Tax?

Born form the Emergency Economic Stabilization Act

During the financial crisis, of October of 2008, Congress passed the Emergency Economic Stabilization Act. Part of that Act created the Troubled Asset Relief Program (TARP) with a $700 billion fund for loans to banks. As part of TARP, the banks borrowed $248 billion. Approximately $199 billion of that amount has been repaid.

Auto and Insurance Companies

However, TARP was also extended to auto companies and auto-financing entities. In addition, because of the fear of collapse of the entire system if securities insurance funds went under, the government funded a loan/guarantee of over $100 billion to financial giant AIG. The total distribution to General Motors, AIG, CIT and related entities was $244 billion. Most of this amount is still unpaid.

Treasury Requirement

Part of the Emergency Economic Stabilization Act was a requirement that Treasury eventually determined the best means for the government to be repaid the TARP funds. As a result, Treasury Secretary Timothy Geithner has proposed a 0.15% tax on the covered liabilities of banks with over $50 billion in assets. Secretary Geithner appeared on May 4, 2010 before the Senate Finance Committee to explain the administration proposal. He noted that it is appropriate for the banks to pay for TARP. The TARP Program put out “a financial fire” and the banks benefited from it. Sec. Geithner stated that the purpose of “the Financial Crisis Responsibility Fee proposed by President Obama in January is to make sure that the direct costs of TARP are paid for by the major financial institutions, not by the taxpayer.”

Small Business Unharmed

In response to concerns by senators that a TARP tax may cause lending to small businesses to be reduced, Sec. Geithner responded, “Small businesses would not be harmed by the tax for the simple reason that it leaves untouched 99% of American financial institutions.” The tax on large banks is estimated to raise approximately $90 billion over 10 years. The cost of TARP is currently estimated by the Congressional Budget Office to be approximately $110 billion.

Assessment

Sen. Max Baucus (D-MT) indicated that a TARP tax is “inevitable.” He stated, “We need to understand the best way to design the tax so that it’s fair and achieves its purpose. We need to understand who should pay the tax. And we need to understand what effect the tax would have on small businesses and the economy.”

Predictably, the American Bankers Association opposed the tax. Their representative James Chessen responded, “Had the TARP been limited to the banking industry, there would be no losses on that program.” The ABA maintained that it was not fair to tax the banks because government losses on TARP will be primarily related to the auto companies and other non-bank recipients of TARP loans.

Overhauling Health Care Oversight

Union Pressure Exacts a Toll on California Bill

A California Senate committee postponed a vote last Monday on legislation [1] to speed up the investigation and discipline of health professionals accused of misconduct, even as the bill’s author removed some provisions opposed by labor unions.

Two Bills Advance

Separately, the committee advanced two other bills to address problems raised in articles last year [2] by ProPublica and the Los Angeles Times. One bill [3] would require that temporary staffing firms perform certain checks before sending nurses to hospitals; the second [4] would mandate that substance-abusing health workers be barred from practice if they test positive for drugs while participating in a treatment program. The bill to overhaul the disciplinary process for California’s nearly 1 million health professionals is the most contentious. It would require that employers report workers who are fired or suspended for serious wrongdoing, a mandate in place in 36 states already. Some California licensing boards already require this, but the Board of Registered Nursing [5], which oversees more than 350,000 nurses, does not.

A Controversial Provision

Another controversial provision could result in closure of state-run recovery programs for substance-abusing health professionals. The Senate Business, Professions and Economic Development Committee heard testimony—including objections from unions and professional trade associations—during a hearing Monday but postponed a vote until Thursday. Even if the legislation clears the committee, it still must get through several others and the full Assembly and Senate.

Critical Response

In response to critics, the bill’s author, Democratic Sen. Gloria Negrete McLeod [6], agreed to remove a provision that would have allowed the director of the state Department of Consumer Affairs to quickly suspend the license of a practitioner who is believed to pose an imminent risk to the public.

She also stripped language to allow licensing boards to deny a license based on a mental or physical disability.

Negrete McLeod said the bill is urgently needed to keep dangerous and incompetent nurses from being able to continue practicing, sometimes for years, regardless of misconduct allegations. “There was enough blame to go around that we could all get painted with the same brush,” she said. Critics said the changes made the bill better, but the still oppose major elements. It is unclear whether the measure has the votes to pass.

Leland Yee

Democratic Sen. Leland Yee said the bill may have an “overemphasis” on consumers at the expense of the rights of professionals. “Some people may say we can throw away” the due process rights of professionals, Yee said. “But that’s not the way we do things in this country.”

Assessment

Finally, Negrete McLeod said she found arguments against the bill “ironic.” Each of the representatives of the various professional groups had “averred that only 1 percent” of their members were getting into trouble. If that is true, she asked, why was there such a “hue and cry” over the proposed reforms?

The Financial Crisis Panel’s Huge Task

The Financial Crisis Inquiry Commission, a 10-person panel tasked with investigating the causes of the financial crisis, is gearing up for when it shortly begins a second round of public hearings with financial executives, regulators and experts.

The Panel’s Mission

As has been reported [1] but bears repeating, the panel has a big job [2] and a small budget. The commission’s chairman, Phil Angelides [3], has pointed out that the Lehman Brothers examiner’s report—an investigation into what happened with a single company—cost $38 million and took 15 months. By comparison, the commission, whose job is to investigate the entire financial crisis for the public record, has an $8 million budget and Dec. 15 deadline for reporting its findings to Congress. Recent reports say the crisis panel has been beset with internal disagreements and staffing delays [4].

Deceptive Accounting by Bankers?

As part of its formidable to-do list [2], the panel has promised to look into the bankers [5], into the extent of deceptive accounting [6] ($) at financial firms, into the mortgage bubble [7] and into subprime lending at Citigroup [8]. Among the people to be questioned this week are former Fed chairman Alan Greenspan, former Citigroup CEO Chuck Prince and former Citi chairman Robert Rubin—who as Treasury secretary during the Clinton administration played a “key role in financial deregulation [9].” Current and former heads of the Office of the Comptroller of the Currency [10] will also face questioning, as will former executives from mortgage giant Fannie Mae and two former Fannie regulators.

Being Alan Greenspan

As for what to expect: Alan Greenspan has laid out his position on the financial crisis in a paper for the Brookings Institution (read the summary [11]). In recent interviews [12] he has stressed that the problem is not the markets, but that no one understood the risks that were being taken enough to step in. “We have to work our way back to understanding what went on,” he told ABC News—troubling, coming as it does from someone who spent more than two decades as the nation’s chief financial regulator. He also stressed “more required capital” for financial firms, an idea that seems to be popular in principle [13], but only so long as regulators have discretion and specific numbers aren’t written into legislation [14].

Assessment

Former Citi execs Prince and Rubin will likely be in the headlines as well, as the panel questions them on the risky lending practices that led to $45 billion in government bailouts for Citigroup (see our bailout page on Citi [15]). In 2007, shaking off concerns about the mortgage lending boom, Prince famously told the Financial Times, “When the music stops, in terms of liquidity, things will be complicated. But as long as the music is playing, you’ve got to get up and dance. We’re still dancing.”

Tax Freedom Day

Each day the Tax Foundation reports the date when Americans have on average worked enough to cover taxes. For the year 2010, today the 99th day of the year, or April 9th, is Tax Freedom Day.

2007’s Comparison Triad

Tax Freedom Day this year is two weeks earlier when compared with 2007. This occurs for three principal reasons. First, the recession has reduced tax revenue. Second, there were temporary tax cuts for 2009 and 2010. Third, in 2010 the partial loss of itemized deductions and the exemption phase out for higher-income taxpayers are both eliminated. In addition, there is at present no estate tax.

Now Consider the Debt Burden

However, if the $1.3 trillion deficit is considered, the picture changes substantially. If the government were to collect sufficient revenue to balance the budget, then Tax Freedom Day would be May 17, 2010.

Assessment

As the $1.3 trillion US budget deficit is financed; all taxpayers will either be required to pay interest on that amount or eventually pay off that debt. With the deficit considered, Tax Freedom Day would be the latest since World War II.

According to Charles Jaffe, a Dow Jones Columnist, it’s one thing to say you want to do the right thing, and something altogether different to actually do it.

Actually, I contacted Chuck early in my journalistic career, about a decade after receiving my certified financial planner designation, and right before he commenced matriculation for same.

The ME-P Position

For years, we at the ME-P, along with some enlightened physician-investors, and a few financial advisors, have suggested that mutual fund managers be paid only when they make money for shareholders. Yet, when a fund company establishes a performance fee and puts its money where its mouth is, the idea gets shot down in the marketplace.

Otherwise, a pure assets-under-management fee schedule approach encourages “asset gatherers” [aka: salesmen/women]; not true asset managers.

IOW: Industry performance standards are quite low, yet we paradoxically pay managers to “beat” the averages, but measure them based on those same “averages.”  And, we still pay regardless of outcomes; in one way or another.

Assessment

Some of the fault, of course, lies with SEC rules and regulations as well as some investors. But, as funds like TFS Capital kill off the most aggressive performance-based fee structure in the performance based business, it sends us all notice that true performance-based fees may be dead

Hayes Announces Appointment of Dr. Susan Levine

Hayes, Inc., the nation’s leading health technology research and consulting company, today announced that Susan A. Levine, DVM, MS, PhD, Vice President of Health Technology Research and Consulting, has joined the Medicare Evidence Development and Coverage Advisory Committee (MEDCAC).

The MEDCAC was established in 1998 to provide independent guidance and expert advice to the Centers for Medicare & Medicaid Services (CMS) on specific clinical topics. Dr Levine will serve a two-year term beginning January 1, 2010.

About MEDCAC

MEDCAC is utilized by CMS to supplement its own research and allow for additional expert and public input on coverage topics, including those that are highly complex or have a major potential impact on the health of beneficiaries and/or the Medicare program itself. The MEDCAC reviews and evaluates medical literature and technology assessments to determine reasonable and necessary applications of medical services and technology. Advisory in nature, MEDCAC provides an unbiased, external evaluation of the strength of available evidence in a public forum. The committee also provides expert clinical and technical review regarding conclusions about the net health outcomes of a technology under consideration.

Interview

MEDCAC members are valued for their background, education, and expertise in a wide variety of scientific, clinical and other related fields. “I am honored to be invited to work with this prestigious group of professionals,” says Dr Levine. “In order to improve the quality of healthcare in the United States, decisions about policy, coverage, utilization, and personal healthcare must be grounded in scientific evidence.”

Dr Levine has extensive experience in health technology assessment, clinical trial methodology and data analysis, and comparative effectiveness reviews, and has held a senior management position at Hayes, Inc. for the past 13 years.

Previous Career

Before joining Hayes, Dr Levine earned a PhD in Pharmacology and Doctor of Veterinary Medicine degree from Cornell University, and completed a surgical residency at the University of Wisconsin, School of Veterinary Medicine. She has held faculty appointments at the University of Wisconsin-Madison, Johns Hopkins University School of Medicine, and Mayo Graduate School of Medicine.

Policymakers working to reduce the number of persons without health insurance sometimes note the significant number of uninsured who are already eligible for public coverage but not enrolled.  In this essay, Dr. Genevieve Kenney of the Urban Institute explores reasons behind this phenomenon and describes steps that can be taken to facilitate and encourage enrollment among eligible individuals. She stresses the importance of multi-faceted approaches that promote program awareness, simplify eligibility determination, and automate enrollment and renewal processes. As our country considers expansions in publicly funded coverage, these lessons should be top of mind in order to maximize program enrollment and reach as many uninsured people as possible.
