As higher standards of living and better health care are reaching more parts of the world, the rates of fertility — and population growth — have started to slow down, though the population will continue to grow for the foreseeable future.

The Forecasts

U.N. forecasts suggest the world population could hit a peak of 10.1 billion by 2100 before beginning to decline. But, exact numbers are hard to come by — just small variations in fertility rates could mean a population of 15 billion by the end of the century.

So, watch this video as the global population explodes from 300 million to 7 billion.

US Healthcare Expenditures Reaching Unsustainable Levels

Expenditures on healthcare in the United States continue to increase and are rapidly reaching unsustainable levels. Pressures by businesses, households and the government to address these escalating costs and ensure high-quality healthcare are multiplying.

This is the first in a series of six essays that examine the challenges facing health plans and the ways that network design can unlock affordable care by aligning products, care models, and reimbursement.

Health insurance companies are faced with addressing a rapidly changing healthcare environment on multiple fronts. These changes are being driven by the goal of achieving a more affordable, higher quality healthcare system. Shifting market needs, increased regulatory initiatives, and a demand for administrative efficiency are requiring innovative approaches to unlocking affordable care. These pressures are originating from key healthcare stakeholders—employers, members and the government (Figure 1).

Employer Pressure

As the competition for the group insurance market increases, health plans need to respond to employer demands for products that deliver greater value. Delivering high value requires products which are tailored to the health of the employer’s specific population and emphasize wellness and prevention. An employer that can offer benefits and programs tailored to meet their employee needs can both improve their workforce productivity and optimize their healthcare spend. The employer’s insistence for reduction in premiums and decrease in the rate of premium growth is challenging health plans to develop more innovative strategies.

Consumer/Member Pressure

With the passage of the Patient Protection and Affordable Care Act of 2010 (PPACA), the Congressional Budget Office (CBO) estimates (Figure 2) that approximately 32 million more individuals will require access to healthcare services. This represents a significant increase in the number of new healthcare consumers at a time when health insurance companies are required to guarantee issue and re-newability of coverage. Steering this influx of new members to the right care teams will be a very critical core competency for health plans to develop. It is one of the few risk management tools left in the plan’s arsenal in a guaranteed access world. The growth of the individual market is also being accompanied by an increase in member financial responsibility. Members are increasingly demanding greater transparency into their provider quality, performance and cost information.

Government/Regulatory Pressure

Evolving healthcare regulation puts still more pressure on health plans. New regulations within the PPACA Section 9016, stipulate an 80% MLR cap for small groups (fewer than 100 lives) and an 85% Medical Loss Ratio (MLR) cap for large groups (more than 100 lives). These regulations also cap the percentage of revenues that can be earmarked for operational and administrative expenses at 15-20%. This poses a unique challenge for health plans; it requires plans to innovate in the areas of products, care models, and reimbursement designs without increasing the administrative and operational overhead.

There are roughly eighteen additional PPACA provisions that put further pressure on health plans by promoting increased collaboration (sections: 6301, 4201, 3027, 3011, 3021, 10333, 3022, 3024) and accountability (sections: 2705, 3006 & 10301, 3001, 3025, 2706, 2704, 3023, 3004, 3008 and 3002). The Bureau of National Affairs best summarized these provisions by stating,

“The comprehensive provisions in the act regarding payment and delivery reform reflect both the payment system continuum—from fee-for-service to bonus incentives for quality to bundled payments to partial and full global payments as well as the delivery system continuum—from independent clinicians and hospitals to small group practices to multi-provider networks to partially or virtually integrated organizations to fully integrated systems with common ownership and employment.”

These demands mean that health plans need to offer new high-value products that incorporate outcome-based reimbursement to drive quality outcomes and not pay for potentially avoidable costs.

According to studies by the Robert Wood Johnson Foundation and Prometheus Payment (2009), “Up to 40 cents of every dollar spent on chronic conditions and 15 to 20 cents of every dollar spent on acute hospitalization and procedures are attributable to potentially avoidable complications (PACs).”

With evidence like this health plans are taking a new, hard look at when and how care is delivered.

Although each video–five in all–clearly has high entertainment value (for example, you’ll never be able to listen to Johnny Cash’s “Ring of Fire” in the same way again), some do have messages that go beyond the wackiness portrayed on the parody surface … to reveal a deeper more insightful truth or value!

Assessment of the Parody

“Many a true word is spoken in jest” and “Some truths, too painful or too likely to provoke, can be spoken only when the listener has been disarmed by laughter” are proverbial truths.

The idea appears to have been recorded first by Geoffrey Chaucer with the line, “A man may seye full sooth (truth) in game and pley” in his “The Canterbury Tales” (circa 1387).

In “King Lear” (1605), William Shakespeare wrote,”Jesters do oft prove prophets”; and some years later, the modern version was rendered in the “Roxburghe Ballad” (circa 1665): “Many a true word hath been spoken in jest.”

National Opt Out of the Airport Scanners Day

Today is the day that ordinary citizens stand up for their rights, stand up for liberty, and protest the federal government’s desire to virtually strip us naked or submit to an “enhanced pat down” that touches people’s breasts and genitals. You should never have to explain to your children, “Remember that no stranger can touch or see your private area, unless it’s a government employee, then it’s OK.”  

Goals and Objectives

The goal of National Opt-Out Day is to send a message to our lawmakers that we demand change.  No naked body scanners, no government-approved groping. We have a right to privacy and buying a plane ticket should not mean that we’re guilty until proven innocent. This day is needed because many people do not understand what they consent to when choosing to fly.

The Details

Here are the details:

Who?

You, your family and friends traveling by air on Wednesday, November 24th 2010 

What?

National Opt-Out Day.  While the government doesn’t always like to advertise this, you have the ability to opt-out of the naked body scanner machines (AIT, or Advanced Imaging Technology, as the government calls it). All you have to do is say “I opt out” when they tell you to go through one of the machines.  You will then be given a pat down.

Where?

At an air-port near you.  

When?

Wednesday, November 24, 2010.  That’s right: November 24 – one of the busiest travel days of the year! We want families to sit around the dinner table, eating turkey, talking about how a government employee molested them at the airport.  We hope the outrageous experience then propels people to write their Member of Congress and the airlines to demand change.  

Why?

We are sick of “security-theater.”  These naked body scanners do not make us a more secure nation. In fact, the scanners, which use radiation, may not even be safe for our long-term health. The government should not have the ability to virtually strip search anyone it wants. Why should a government employee get to see a naked scan of a passenger, and do who knows what in the back room while viewing that image?   We have already heard stories of TSA officials laughing at small genitals and making certain women go through the machines or taking off extra clothes, reducing them to tears. This is absolutely sick behavior. If you don’t like it and don’t want to be virtually strip searched, then too bad says the government. To try and make everyone comply with the naked body scanners, the government has made the alternative worse! With their enhanced pat downs, TSA now touches the genitals and private areas of men, women and children with the front of the hand! We do not believe the government has a right to see you naked or feel you up just because you bought an airline ticket. There are better, less invasive security measures that can be taken.

How?

By saying “I opt out” when told to go through the bodying imaging machines and submitting to a pat down. Also, be sure to have your pat down by TSA in full public – do not go to the back room when asked.  Every citizen must see for themselves how the government treats law-abiding citizens.

Reaching for a patient chart in the ER one night, a young physician is suddenly overwhelmed by a strange mixture of panic, anger and fear. But, such emotions are unacceptable for a doctor.

Podcast: How the Health Care Bill Comparison News Application Came Together

Last week, Olga Pierce and Jeff Larson created a side-by-side comparison of the health care bills [1] to help people see the exact changes in the legislation. Larson developed a news application that highlighted the changed, added or deleted provisions of the bill and Pierce had the unenviable task of going through the 2,000-plus page bill to decipher what the changes were.

One interesting idea is a patent for 3-Dimensional head-tracking. Instead of using a mouse and keyboard, Mac users simply move their head or body to control an image on screen. Some think this technology would perfect for healthcare.

Recently, we caught up with Houston Neal – of Software Advice – who thought our ME-P readers would like to see their new podcast on eHR stimulus funds. In-as-much as they are still hearing from doctors who want to know how to take advantage of the stimulus, they’ve teamed up with the Chairman of HIMSS to help answer questions via webinar. The final clip is now live on the blog.

And the Question … Is?

After talking with hundreds of physician practices each month, their biggest question seems to be: “What does the economic stimulus package mean for me?”  

Of course, practices understand that up to $45 billion is allocated to provide incentive for physicians to adopt eHRs. However, many questions remain about how and when providers will receive stimulus funds.

What is ME-P TV?

The emergence of Internet video is creating new opportunities to provide richer, more effective information on our core subject areas and more than 50 related topical channels. Internet video gives anyone the ability to create a personal TV channel to distribute information on demand. We are creating the opportunity for you to participate in the Internet video revolution. ME-P TV opens the door to real financial advisors, medical management consultants and health care providers to share their views and opinions with us; each and every day.

Why would I want to be on ME-P TV?

By participating in ME-P TV, you help us create a useful resource for readers and subscribers, and thereby enhance our product. At the same time, we share these video materials with you to market your ideas, products or services. You are free to use these video materials in any way that you want, including on your own website, mobile device, wiki or blog; or on a DVD to distribute to your referring clients, advertisers and patients, or on your local news station. It’s your choice.

Multiple Formats Available

One format is the simple one-on-one interview between a host [Dr. David Edward Marcinko or Hope Rachel Hetico; RN, MHA] and the guest. We use a format similar to the “Charlie Rose” style – two participants sitting across from one another at a table. Generally, each interview focuses on a single topic area. Each informal interview lasts about 15 to 20 minutes. The structure of each interview is not necessarily consistent to keep interest high and energetic.

You may also send in video material that you have previously recorded for your own purpose. Whatever you would like to get on file, send it in and we will post for free. We do reserve the right of refusal, of course. Once published, the video is available to incorporate into your own website and/or for syndication to any other websites.

ME-P TV is a partnership between you and us. Once your message is produced and available on the Internet for distribution, this message is your asset to use as you like. Embed that message on your financial advisory website, your hospital or medical practice website, burn it on a DVD for physical distribution, send it to your friends, colleagues, clients, prospects and patients via email, or make it available on your local news. We will be hard at work making the message available to other health economics and medical management portals, as well. In addition, we will continue to build a portal devoted to integrating financial planning with medical practice management for all physicians and advisors.

Let’s Think about Vitality GlowCaps for a Moment

Vitality Inc, is a new firm that reports to address the billion-dollar adherence problem for pharmaceutical brands, retail pharmacies, and healthcare providers with a simple device — an Internet-connected pill cap.

What it Is

Vitality GlowCaps illuminate, play a melody, and even ring a home phone so patients don’t forget to take their pills. They can send weekly emails to remote caregivers, create accountability with doctors through an adherence report, and automatically refill prescriptions. Vitality reports to improve medication adherence, health, and peace of mind.

Name Droppers

According to its website, Vitality is currently working with researchers at Harvard Medical School and the Center for Connected Health on a study to measure the impact of the GlowCaps CONNECT system.

Vitality is the only company with a product designed to tackle the combination of factors that conspire to cause non-adherent behavior.

Overkill

Our society is now at the point of paying obese patients to diet and exercise [they don’t seem to recognize the benefit], paying public school children to study [they don’t seem to recognize the benefit], and now using internet enabled technology to help patients remember to take their own medication [they don’t seem to … yada, yada, yada].

A Reasonable Query?

But, may one reasonably ask; is this admittedly “very cool” technology overkill? THINK: RFID tags for wrong extremity surgery; when common sense and a magic marker might do just as well? Sure, there may be some modicum of benefit here for elderly patients and select other reasons. But, does the marginal cost outweigh the marginal benefit? Or, is this technology really a solution in search of an exaggerated “problem” that just may involve slack personal responsibility? And, most importantly, who will pay for it?  

Assessment

Vitality Inc, leverages deep expertise in customer research, wireless consumer electronics, web services and behavioral psychology. Vitality’s patent pending solution is said offer each patient the optimal mix of intervention, feedback, reminders, accountability, education and incentives to improve their ongoing medication adherence. But, does it really improve health, and at what cost? Can’t we solve the “problem” of pill non-compliance cheaper and easier?

stk323168rknThere is a major controversy in the modern healthcare community over eMRs and how to pay for them; or even if they are effective in improving medical outcomes. Of course, by eMRs we mean interoperable medical records that span the pan-healthcare ecosystem; and not just the stand-alone digital records that many, if not most, physicians use in their daily practices to some degree or another.

Proponents

As readers of the ME-P are aware, one vocal camp supports certification and eMR industry mandates, standards, and governmental initiatives, etc. The recent $20 billion taxpayer input from the Obama Administration, courtesy of HITECH, further emboldens CCHIT and related wonks.

Opponents

One the other hand, one vocal ME-P opponent is dentist Darrell Pruitt. He and many others believe that current eMRs may be too expensive, unwieldy, and counter-productive. This camp advocates a mix of other data sources, technology processes and doctor/patient education to get us where we need to be in terms of improving medial outcomes; quicker and less expensively.

The eEHR Privacy Debate Continues

According to our colleague Richard Mata; MD, MIS, writing in the premium print-journal Healthcare Organizations [Financial Management Strategies], a critical feature of any healthcare information system [HIS] is compliance with privacy requirements. Of course, the most important compliance regulation is the Health Insurance Portability and Accountability Act of 1996 (HIPAA).

The key here is to have computer systems, terminals, workstations, servers and hand-held systems fully in communication with each other — including the ability to send data outside the fire-walls of the institution; interoperability as needed — while ensuring the confidentiality of protected health information (PHI), which is health information where the person to whom it belongs is identifiable

Federal Privacy Regulations

The federal government required hospital and healthcare entity compliance with HIPAA security regulations since April 2005. Briefly, the following are features of HIPAA which concern HIS:

·         HIPAA presents a unique opportunity for automation of information since it is easier to protect secure information electronically as compared to having a paper chart that can be lost or open in front of patients and visitors.

·         Secure password protection must be in place at multiple levels to ensure that access to PHI is restricted to those who need the information at that time.

·         Appropriate encryption of data is essential for transmission between systems in order to prevent the interception of data.

To date the project has brought together researchers, medical professionals, and game developers to share information about the impact games on medicine.

Additionally, the firms’ GameBase is the most robust and current database of healthy games available – including basic information on every game (company, contact, price, etc.), demos and other downloads, as well as plenty of community feedback, ratings and reviews.

Emotional Pleas Tug at Citizenry Heart Strings

According to Nick Cargo – of the BBC on January 25 – a January 19th episode of BBC’s One’s Panorama, the world’s longest running television documentary show, tackles the dismal state of health care in the United States. Of course, it emotionally parses on the lengths to which our estimated [high-side] 45 million uninsured citizens [only 8 million remain uninsured for 2 years, or more] go in pursuit of care, the pharmaceutical industry’s rigged pricing against the American patient, and the insurance industry’s efforts to deny care whenever possible.
