PART I: Understanding Hospital Information Systems

For the last 30 years, hospital information systems (HISs) have steadily grown in popularity. At present, nearly every acute care hospital in the United States has at least some form of HIS – at a minimum – performing administrative tasks such as patient billing, payor accounting, and employee payroll tracking. Depending on the size of the hospital, an information system may initially cost from several hundred thousand dollars to tens of millions of dollars. In 2005, about $25.8 billion was spent on hospital information technology (IT) in the United States.

By 2010-11 and beyond, it has become critical that such a system produce a positive return on investment (ROI) through patient care quality improvements, increases in organizational efficiency, or enhanced negotiating power with third-party payors and other stakeholders. Some HIS projects, especially more complex undertakings, have been clear failures to the extent that the organization had to abandon the new HIS in favor of the previous manual system. The goal of Part I is to illustrate how hospitals can maximize the chances for successful implementations of HIS while at the same time providing a positive ROI.

PART II: eMRs for the Independent Medical Practice

Many large organizations have a Chief Information Officer (CIO), a Chief Technology Officer (CTO), perhaps a Chief Medical Information Officer (CMIO), or Chief Medical Officer (CMO) that is tasked to do things like figure out the information technology (IT) and computer systems that support clinical and business goals within a healthcare organization. Smaller groups and practices don’t have enough resources to have dedicated staff for such a purpose so the selection, launch and implementation of eMRs will fall onto the physician staff or other senior leadership at a practice … Are you ready?

PART III: Health Information Technology Risks

Your clinical and business data and how it is used is vital to your medical organization.  Information technology (IT) and its infrastructure must therefore be protected through careful security policies.  These policies are guided in part by the risk inherent in your health organization’s IT infrastructure, its competitive strategy, and its asset and risk management policies.  It is important, however, not to over focus on security implementation so that you overlook the clinical operation management and patient-centered care in your healthcare facility.

There are two parts to the security equation: the first involves meticulous inventory of hardware, accessories, and software and the other involves establishing secure IT policies and procedures.  Management systems like those developed by the International Standards Organization (ISO) and the National Institute of Standards and Technology (NIST) serve as very functional models that can be applied in the clinical setting.

Part III of this CD-ROM  reviews the model codes that offer templates on how to put protections in place and the regulations that have been designed to ensure information security.  It also discusses the business continuity and risk management systems you need to keep your organization functioning in case of security and/or privacy breaches. It also offers interpretation and recommendations in making choices as they relate to the IT systems and discusses good healthcare security and privacy practices.  For example, the use of role-based access in an electronic health record (EHR) is not mandatory, but that does not mean that it cannot be implemented. In fact, it is usually desirable.  Many organizations will used a modified role-based system to limit the amount of protected health information (PHI) that one needs to access.

On this 2010 Memorial Day weekend, please allow us to directly reflect for a moment on the decline of the healthcare, banking and financial services industry in America. And; then somewhat indirectly comment on the hopeful emergence of the web 2.0 phenomena of which we all are a part. The competitive applicability to these sectors should be appreciated by the insightful ME-P reader.

Collapse of Command and Control Monopolies and Oligarchies   

Old monopolies everywhere are crumbling because of tougher new competitors and the transparency wrought by electronic connectedness. For example, our old newspaper has to compete with the internet, your electric utility company battles low-cost local start-ups, telephone companies must begin installing fiber optic lines to fend off cable companies; and RIAs and fiduciary focused financial advisors [FAs] will supplant BDs and stock brokers in the financial services sector.

The airline industry collapsed a few years ago, the banking industry has just collapsed, and the auto industry is recovering as we pen this post. [We have a particular affinity for the auto sector however, as the son of a UAW member and step-daughter of Michiganders]. Regardless, the rush to more intense competition cannot be stopped. As a doctor, FA or other business competitor; you either keep pace or get crushed by quasi-oligarchic organizations like the American Medical Association [AMA], American Podiatric Medical Association [FPMA], American Dental Association [ADA], American Osteopathic Medical Association AOMA], Financial Planning Association [FPA], Certified Financial Planner Board of Standards [CFP BoS], College for Financial Planning [CFP] or the National Association of Personal Financial Advisors [NAPFA], etc. What have they, and Wall Street, done for you … lately? Health Insurance Companies

In the last-generation of health insurance companies and related fraternal medical organizations, patients exercised great control over physician selection, had quicker access to specialists and encountered fewer restrictions on care. The reverse was true with financial services. But, because of advancing technology, aging demographics, intense R&D, global manufacturing, and escalating domestic HR costs – competitive market forces against traditional and structured staff model managed care companies – many industry analysts [like us] predicted growth would decline [Yes, greed was also involved as healthcare was presumed a recession-proof sector; and didn’t we all own behemoth big-pharma and HMO stocks in our 401-K, and 403-B plans]? But now, many former stock-brokers and FAs are going rogue; er – independent!

Next-Gen with “Fly”

Fortunately, a new generation of enlightened physician and FA entrepreneurs is coming “out-of-the-shadows” as new-wave web 2.0 corporations and RIAs are becoming more flexible, competitive and market responsive. Simultaneously, monolithic and collectivist political ideas keep trying to regulate the medical and financial services workplace with rules, regulations and contracts to control entire populations. Yet, in the new healthcare economy, this new generation of doctors and FAs with “fly,” is headed toward more competition; not less – with more collaboration with patients and clients – regaining self autonomy.

Physician and FA Advocates

Meanwhile, as medical professionals, FAs and patient advocates, we must all choose between staying flexible to ride out tough times – or – adopting a hard, brittle line that will crack under the pressure of competition. We know where we stand at the ME-P, do you?

Flexibility and Virtual Reality

In recent years, many large corporations and top-down business models were not market responsive and change was not inherent in their DNA. These traditional organizations represented a rigid or “used-to-be” mentality, not a flexible or “wanna-be” mindset; according to business columnist Alan Webber. Some financial advisory corporations, and today’s emerging health 2.0 initiatives, may possess the market nimbleness that cannot be recreated in a controlled or collectivist [nationalistic] environment. And so, going forward, it is not difficult to imagine the following new rules for the new financial and virtual medical ecosystem.

[A] Rule No. 1

Forget about “SEC suitability and FINRA rules”, large office suites, surgery centers, fancy equipment, larger hospitals and the bricks and mortar that comprised traditional medical practices or financial product delivery systems. One doctor or niche focused FA with a great idea, good bedside manners or competitive advantage, can outfox a slew of public servants, the AMA, SEC, ADA or FINRA “faux copy-cat examiners”, while still serving the public – and patients – and making money. It’s now a unit-of-one economy where “Me Inc.”, is the standard. Physicians and FAs must maneuver for advantages that boost their standing and credibility among patients, peers, payers, customers and clients. Examples include patient satisfaction surveys; outcomes research analysis, evidence-based-medicine, physician economics credentialing and true integrated fiduciary-focused financial planning.

However, we should also realize the power of networking, vertical integration and the establishment of virtual RIAs or medical practices, which come together to treat a patient, or help a client, and then disband when a successful outcome is achieved. Job security is earned with more successful outcomes; not necessarily a degree, automatic AUMs, certifications or onsite presence. In fact, some competition experts, like Shirley Svorny PhD, a professor of economics and chair of the Department of Economics at California State University, wonder if a medical degree is a barrier – rather than enabler – of affordable healthcare.

Others even presume the establishment of virtual medical schools and hospitals, where students and doctors learn and practice their art on cyber-entities that look and feel like real patients, but are generated electronically through the wonders of virtual reality units. The same can be said for the financial services industry, although much farther down-line given its current slow rate of real education and quasi-professional acceptance.

[B] Rule No. 2

Challenge conventional wisdom, think outside the traditional box, recapture your dreams and ambitions, disregard conventional gurus and work harder than you have ever worked before. Remember the old saying, “if everyone is thinking alike, then nobody is thinking”. Do collective-nistas and nationalized healthcare advocates react rationally; or irrationally? [THINK: Wall Street, medical unions]

[C] Rule No 3

Differentiate yourself among your healthcare and financial advisory peers. Do or learn something new and unknown by your competitors. Market your accomplishments and let the world know. Be a non-conformist. Conformity is an operational standard and a straitjacket on creativity. Doctors and FAs should create and innovate, not blindly follow organization or political “union” leaders [shop stewards, BDs, etc] into oblivion.

[D] Rule No 4

Realize that the present situation is not necessarily the future. Attempt to see the future and discern your place in it. Master the art of the quick change with fast but informed decision making. Do what you love, disregard what you don’t, and let the fates have their way with you. Then, decide for yourself if you are of this ilk – and adhere to any of the above rules? Or, just become an employed [government, BD] doctor or FA shill. Just remember that the political party, or monopoly that can give you a job, can also take it away [THINK: LB, ML, Wachovia, national healthcare, etc].

Finally, on this Memorial Day weekend, consider that life and career is a journey, and that in this country we have the choice to ponder or pursue any, and all of the above options, and more. We have the ability to think, cogitate and ruminate, as we have done here today. So – please – thank those who have helped turn this idealistic philosophy, into pragmatic daily reality.

Appreciating Parallels for the Emerging US Healthcare System?

Aim of the Conference

In the late 1990s, the Dutch government started to experiment with ‘regulated competition’ in social health insurance. A milestone was the new Health Insurance Act in 2006 introducing a compulsory health insurance scheme for the entire population, carried out by (for-profit) health insurers, contracting individual and institutional health professionals. Safeguarding equal access, the new health insurance scheme introduced several preconditions like compulsory insurance, a basic benefit package, the prohibition of risk selection, a risk-equalization fund, etc. The idea of competitive health insurance was combined with deregulating hospital planning and liberalizing health care tariffs. 

Merit Considerations

In the new scheme medical need is still decisive in health care access decision-making, but merit-considerations are becoming important too. Shortening waiting times, priority arrangements were considered and/or introduced, based on non-medical criteria. Simultaneously, in terms of financing, health status has become important due to own payments-arrangements, limited insurance package options, etc. At the same time, health status disparities due to socioeconomic inequalities seem to be increasing.

Under these circumstances, confronted with increased health spending, we can expect the “R” word becoming more eminent in the Dutch health care debate.

Relevant Rationing Questions

Emerging relevant questions are:

  • Who is responsible for rationing (markets, governments, bureaucrats, MDs or others)?
  • How does it function (explicit or implicit)?
  • What are relevant selection criteria (QUALYs, DALYs, health status, sex, age, etc)?
  • To what extent is current rationing just?
  • What can be done to make it more just?
  • How will health care rationing affect equal access to health care?
  • What is the relationship between rationing and differences in health status etc?

There is a wealth of literature in political theory, as well as in health care policy, economics, social medicine and law addressing these issues. What is needed is a consideration of the values involved, and the impact of, policy decisions on the expression of these values.

The Erasmus Observatory

Therefore, the Erasmus Observatory organized an international conference to discuss health care rationing from a wide range of perspectives.

ME-P Joins Up with SocialVibe

What it is – How it works

SocialVibe is a new networking website with over 1,000,000 members. It was founded by Joe Marchese, Brandon Mills and David Levy, and aims to reward publications like the ME-P through donations to their charity of choice.

How it Works

After joining SocialVibe, in addition to creating a personal profile, users are asked to select their favorite cause to support and to choose a sponsor. Once they have done so, they can earn points for themselves, which can be redeemed for a variety of different perks and money for their respective charities by posting their “badge” to another social networking site. The more a SocialVibe user’s networking site is visited, the more points they are able to earn.

The Badge

A badge is defined as “a distinguishing emblem or mark worn to show membership or achievement.”

The SocialVibe badge brings members together with their favorite brand and charity. The badges are customized to be the member’s own [ie, ME-P] but all consist of their brand sponsor logo, cause logo, member profile picture and a personalized message. The Time Costs of Internal HIPAA Complaints

On Hospital Compliance

The privacy regulations of HIPAA require that each hospital have an internal process to allow an individual to file a complaint concerning the covered entity’s compliance with privacy policies and procedures. This requires hospitals to designate a contact person to be responsible for receiving and documenting the complaint as well as the disposition.

A formal response to the person is not required as part of this rule; therefore it is estimated that each complaint, even though rare, will take ten minutes to document.

Recent Data

Recent data reveals that the most frequent complaints received either by hospitals or ultimately by DHHS include the following:

  • impermissible use or disclosure of individual PHI (most occurrences were curiosity or accidental, yet were reported);
  • lack of safeguards to protect PHI;
  • refusal or failure to provide an individual with access to or a copy of his or her record;
  • disclosure of more information than is minimally necessary; and
  • failure to have the individual’s valid authorization for a disclosure that requires one.

Assessment

Most hospitals have documented and logged such complaints; have reviewed the situation; and have resolved the problem internally.

What is ME-P TV?

The emergence of Internet video is creating new opportunities to provide richer, more effective information on our core subject areas and more than 50 related topical channels. Internet video gives anyone the ability to create a personal TV channel to distribute information on demand. We are creating the opportunity for you to participate in the Internet video revolution. ME-P TV opens the door to real financial advisors, medical management consultants and health care providers to share their views and opinions with us; each and every day.

Why would I want to be on ME-P TV?

By participating in ME-P TV, you help us create a useful resource for readers and subscribers, and thereby enhance our product. At the same time, we share these video materials with you to market your ideas, products or services. You are free to use these video materials in any way that you want, including on your own website, mobile device, wiki or blog; or on a DVD to distribute to your referring clients, advertisers and patients, or on your local news station. It’s your choice.

Multiple Formats Available

One format is the simple one-on-one interview between a host [Dr. David Edward Marcinko or Hope Rachel Hetico; RN, MHA] and the guest. We use a format similar to the “Charlie Rose” style – two participants sitting across from one another at a table. Generally, each interview focuses on a single topic area. Each informal interview lasts about 15 to 20 minutes. The structure of each interview is not necessarily consistent to keep interest high and energetic.

You may also send in video material that you have previously recorded for your own purpose. Whatever you would like to get on file, send it in and we will post for free. We do reserve the right of refusal, of course. Once published, the video is available to incorporate into your own website and/or for syndication to any other websites.

The dynamic of syndication makes the Internet and the World Wide Web the most powerful media distribution engine the world has ever known. This dynamic is fueling the transition of broadcast television into a new entity that combines the richness of video media with the convenience of “download on demand” to create a new way of accessing and distributing information. The Internet video revolution is creating the mechanism for anyone to create and distribute video information to as wide – or as narrow – an audience as necessary to accomplish your professional goals.

The concept is simple:

ME-P TV Helps you accomplish these Tasks

  • You provide the message.
  • We produce the channel of Internet distribution.

ME-P TV is the “contextual” portion of solving the distribution equation. Successful Internet distribution requires both content and context. Content is a critical “necessary but not sufficient” element. Providing an appropriate context to serve as a “container” to aggregate and increase the value of the content is also necessary. A growing collection, or library, of similar content is more valuable than isolated, individual offerings. ME-P TV is the container for your content with a “home” on the Internet from which to begin the syndication process. We represent a niche portal that gives us the ability to aggregate an audience to generate the attention necessary to be able to offer this service to you.

A Partnership

ME-P TV is a partnership between you and us. Once your message is produced and available on the Internet for distribution, this message is your asset to use as you like. Embed that message on your financial advisory website, your hospital or medical practice website, burn it on a DVD for physical distribution, send it to your friends, colleagues, clients, prospects and patients via email, or make it available on your local news. How Doctors Get Paid

Just before the Christmas Holidays, I flew up to Boston at the invitation of a pharmaceutical company to lead a managerial workshop entitled: “How Doctors Get Paid” [Treatment is only the beginning in the Changing Billing and Medical Reimbursement Climate].

Our goal was to inform drug representatives, and their regional managers, what value added information physician offices might expect from the pharmaceutical industry of the future.  

Topics of Discussion

The two hour interactive workshop included team projects, flip chart exercises, a mock role-playing session and the customary [hopefully energetic] ppt presentation. Other topics of discussion included:  

  • Health insurance payment evolution
  • Collapse of Medicare
  • Rise of managed care
  • Medical records documentation
  • ICD-9 and 10, HCPCS, DRGs and CPT® coding
  • ABNs, super-bills and HCFA 150 forms
  • Billing methodologies
  • Healthcare fraud, abuse and related policies
  • Capitation, HSAs, concierge medicine and RACs
  • Futuristic health 2.0 payment mechanisms, and more.

Assessment

Rest assured; these folks were a very knowledgeable and aggressive group; not like your father’s “detail men” of yore! They seek to … talk the talk, and walk the walk, of the Health 2.0 era.

Dr. David Lumsden; MS, MA practices general orthopedic surgery and trauma as a board certified surgeon and partner with Orthopedic and Hand Surgery Associates in Baltimore, Maryland. He completed his training in community health at Towson State University, earned Master’s Degree in Anatomy / Neuroanatomy at University of Maryland/Baltimore and Exercise Physiology at University of Maryland, College Park. He is a graduate of Penn State University School of Medicine. Dr. Lumsden completed his internship and residency at Union Memorial Hospital with associative residency training at Johns Hopkins and the world renowned Shock Trauma Center at the University of Maryland, Baltimore City. 

Our Brief Interview

When I caught up with David during a recent house-call visit, we discussed many things; especially the American Affordable Health Choices Act [HR-3200]. Unfortunately; I did not have my audio-recorder with me. So, here are a few points of interest about him that I jotted down, from memory, in my ever-present reporter’s notebook. No doubt, I missed many more:

  • He became a physician as a career change in mid-life.
  • He has read HR 3200 in its’ entirety.
  • He hired an attorney for HR 3200 interpretation and review.
  • He is for healthcare reform, but against HR 3200.
  • He is against a public health care plan.
  • He is against individual insurance mandates.
  • He does 10-12 house-calls every month.
  • He does not charge MC, MD or VA house-call patients; rarely bills them and/or accepts assignment without balance billing.
  • He regularly operates on same, under similar terms.
  • He does other pro-bono work.
  • He practices defensive medicine.
  • He is for tort reform.
  • He is not a member of the AMA with no plans to join.

While in Washington DC on the second portion of our recent ME-P book “signing and opining” tour, I had the good fortune to visit the gravesite of the noted physician Nathaniel Potter, MD. Dr. Potter was born in Carolina county Maryland in 1770 and died in Baltimore on 2 January, 1843. He graduated from the University of Pennsylvania in 1796, and settled in Baltimore, where he practiced medicine until his death. In 1807, he associated with Dr. John B. Davidge in founding the University of Maryland, School of Medicine where he ultimately served as professor and dean. He died penniless.

About Green Mount Cemetery

Green Mount Cemetery is located in Baltimore, MD. Established in 1839, it is noted for the large number of historical figures that have been interred in its grounds as well as a large number of prominent Baltimore-area families. It retained the name Green Mount when the land was purchased from the heirs of Baltimore merchant Robert Oliver. Green Mount is also a treasury of precious works of art, including striking works by major sculptors like William Rinehart and Hans Schuler. The cemetery was listed in the National Register of Historic Places in 1980.

Assessment

In as much as Dr. Potter was a well know figure to me, I was most pleased at the impromptu visit to his grave. You see, although I attended Temple University because of my future specialty, my first medical school choice would have been at University of Maryland if post-graduate education opportunities had been different at the time. And, I passed the medical school, and the imposing Greek themed Davidge Hall Dome, daily for four years as I rode the number 8 public transportation bus to my undergraduate studies at nearby Loyola University, in Townson Maryland. Of course, the fact that Potter was educated at the University of Pennsylvania School of Medicine, the first in the nation, did not elude me when I worked in its ER as a young medical student in Philadelphia, back in-the-day. University of Maryland was the fifth such medical school in the country.

Off-Road Touring with Dr. Marcinko [Part V]

About the Ship USNS COMFORT

Dateline: August 4, 2009USNS Comfort Canton, Maryland

The official Navy ship USNS COMFORT [“Medical Treatment Facility”] has the primary mission to provide a mobile, flexible and rapidly responsive capability for acute medical and surgical care in support of amphibious task forces. These forces include the Marine Corps, Army and Air Force elements, forward deployed Navy elements of the fleet, and activities located in areas where hostilities may be imminent. Operations are governed by the principles of the “Geneva Convention for the Amelioration of the Wounded, Sick, and Shipwrecked Members of the Armed Forces at Sea”, as of August 12, 1949.

Mission

As a secondary mission of the ship COMFORT is providing a full hospital service asset for use by other government agencies involved in the support of relief and humanitarian operations worldwide. These mission statements are accountable to both the Reduced Operating Status (ROS) and Full Operating Status (FOS) military personnel staffed at National Naval Medical Center, Bethesda Maryland until the ship is activated. However, the ROS personnel’s immediate and number one priority is to fully activate the ship to a FOS Echelon III Medical Treatment Facility within a prescribed 5-day time frame.

Functions

In meeting these missions, the ROS personnel perform the following functions:

  • Serve as the nucleus of the critical core required to execute activations;
  • Develop, test, and maintain systems and procedures to support activation process;
  • Orient and train FOS augmenting staff;
  • Monitor/assess the medical treatment facility’s overall ability to fully activate/perform mission.

Leaders

The ship is commanded by CAPT James J. Ware DDS, with Executive Officer Captain Larnerd, and Master Chief Lohner. The home base is Baltimore, Maryland.  Upon arrival at the dock, the ME-P and I planned to meet onboard with Master David Lieberman. But, an interview was cancelled due to a last minute scheduling conflict. However, we were placed in the competent hands of our civilian tour guide, Shaun B. of Virginia. He noted 70 civilian and 700 military personnel.US Comfort

Assessment

Shaun B, informed me that despite advanced age, my application for a four month tour would be gladly reviewed. And, it is under serious future consideration as my nurse sister is a recent Bronze Star Award winner from a Combat Army Surgical Hospital [CASH] unit stationed in Iraq. I also did my trauma surgery training in Martin Army Hospital, at Fort Benning Georgia, back-in-the-day.

In this month’s edition, we update readers on the results for the Blue Cross Blue Shield universe, and provide product breakouts, summary functional area breakouts as well as expense trends. Cost increases are lower this year than last, though higher if product mix is considered. Twenty-two Blue Cross Blue Shield Plans serving 31.3 million members participated in this year’s benchmarking study.  Growth in Information Systems and Medical Management costs explained more than 40% of the total increase.

I Dislocate a Finger

Fresh off my “runner’s” high from meeting Olympians Shani Davis and Apolo Ohno, the other day, I easily ran an extra couple of miles during my daily training run to stay in shape during my arduous speaking and book signing tour. Thus, invigorated in mind and body, and flushed with surging natural endomorphins, I decided to spend the afternoon at Marquette Mountain watching the downhill mountain bike races.

After introducing myself to Bruce Gustafson, the ambulance driver there on professional standby from the local Marquette General Hospital EMT service, I spent an hour or so marveling at how the racers survived the rocky and treacherous down-hill course. Unfortunately, I was not so lucky, and came very close to requiring Bruce’s expert services.

Taking a TumbleDismay

While climbing down the mounting, running actually, I tripped head-over-heels and commenced wildly flailing and rolling down the rocky terrain, stopping just short of smashing into a large birch tree after 50 feet or so. Bouncing straight back up in an instant, I doubt if nearby spectators even noticed what had happened.  Nevertheless, I was most pleased at my adroit body and still youthful ability to assume a dolphin like roll into the fall, dissipating kinetic energy and avoiding injury. Or, so I thought.

Dislocating a Finger

Unfortunately, my fourth right finger began to throb a few seconds later; and then scream at me. Not much medical acumen was needed to note that something was wrong, as the proximal-inter-phalangeal-joint [PIPJ] appeared laterally deviated forty-five degrees, just as the digital apex was turning blue and the joint began swelling before my eyes. Appreciating the impending neurovascular compromise potential, I grabbed the finger, distracted it, reversed the mechanism of injury, let go, and felt it snap back into place.  I promptly sat down as a diaphoretic wave of cold sweat broke out on my forehead, chest, shoulders and arms. Fortunately, the hypo-tensive episode lasted only a few minutes, and I did not go into shock. A visiting nurse came to the rescue, elevating my feet, monitoring vital signs and offering fluids as I recovered quickly. She was indeed my Florence Nightingale and, as I later learned, a Master’s prepared nurse-executive. I was gratefully taken back to my hotel after refusing further medical assistance or X-rays. The finger was grotesquely swollen the next few days but did not hurt much. Simple contrast baths, passive and active ROM exercises, along with the “tincture of time” which I estimate to be 6-8 weeks based on clinical experience, will hopefully be all that is required for complete recovery. Although, it may be some time before full typing flexibility is returned; assuming a “button-hole” extensor tendon rupture incarcerating, strangling or imbricating the phalangeal head, did not occur.Ambulance DEM

Assessment

In my former clinical medical career, I diagnosed, treated and operated on hundreds of injured fingers and toes; for various acute and chronic conditions, fractures, dislocations and related digital anomalies. I even published several peer reviewed papers on same. Still, my course of action may be considered reckless, by some, and should not be repeated. Always seek medical advice.
