The ONLY way to protect dentists, staff, patients and their families from the risk of fatal COVID-19 infections is to keep the virus out of dental offices. (See graph from the New York Times).
Prediction: If quick and reliable testing is not available soon, within weeks after dental offices re-open for routine dental care – creating aerosols with high speed hand pieces, air/water syringes and ultrasonic scalers – dental offices will justifiably become known as reliable sources of COVID-19 infections, before being closed down again by the state.

If you wear gloves because of Covid-19, and if you don’t take them off properly, you just get everything that was all over the gloves, all over yourself and everything else. As a surgeon for almost two decades, I can tell you that taking gloves off correctly isn’t a trivial thing.

HOW TO REMOVE: Briefly, you want to pinch one glove near the wrist and pull it over your hand so it ends up inside out. Then hold that in your gloved hand and carefully slip the fingers of your bare hand into the top of the other glove, let it turn inside out and cover the balled-up other glove.

An Important Money Skill
“Some days adulting is a pain.” What parent of college-age children hasn’t heard something similar from their kids? The transition from kid to adult is a necessary process toward living a fulfilled and meaningful life, but it isn’t easy or smooth.
This is especially true when it comes to money. Mastering money skills can be a challenge even for older adults.
One of the earliest opportunities to learn adult financial skills comes with renting an apartment. Before you sign that first lease—or any lease—it’s important to understand it. A lease is a legal document that sets out obligations and rights for both landlord and tenant.
One of the most important features of a lease is the length of the agreement. Your lease could be “month-to-month” or for a specific period like a few months or even several years. The most common residential lease terms are six months to one year.
There are pluses and minuses to both types. A month-to-month lease gives the renter the minimum commitment and maximum flexibility. Usually, if you want to move out for whatever reason, you just need to give the landlord a 30-day notice. Unlike a longer-term lease, there is no penalty for “breaking” the lease unless you fail to give even a 30-day notice.
So why wouldn’t a tenant always want a month-to-month lease? Many tenants don’t understand that the flexibility goes both ways. If a landlord chooses to stop leasing the property, finds a tenant willing to pay higher rent, or decides to sell the property, all the lease requires is a 30-day notice for the current tenant to move out. A tenant must accept that risk.
A recent local example concerned 11 house renters who lived on the campus of the Star Academy, a former state-owned property near Custer, SD. Some of the tenants had rented for 14 years with month-to-month leases. When the state foreclosed on the property it gave the tenants 30-day notices to move. This was not received well by the renters, who faced the prospect of immediately having to find new places to live in a town with a limited supply of housing. Fortunately, the governor reversed the decision and gave the renters six months to find new housing.
As shocking and heartless as this move might have seemed to the renters, it was completely within the rights of the landlord, just as it would have been completely within the rights of any of the tenants to do the same.
It’s easy to get lulled to sleep by a month-to-month lease, especially when a tenant has lived in the property for year after year. However, if the prospect of having to vacate your home in 30 days is not appealing, it would be a really good idea to ask the landlord for a longer lease.
Assessment:
Before signing a lease, consider how long you are willing to commit to living in the property. What will best serve your situation? For some, it may be a lease that expires at the end of a school year, or in a year, or even in three to five years if you see no reason that you will need to move anytime soon. Be aware that by signing the lease, you agree to stay and to pay rent until the time is up.
Also understand that, unless the lease specifically states otherwise, neither you nor the landlord are bound to renew when the lease expires. So it’s important to renegotiate a new lease well before the current lease expires.
Before signing any lease, read it carefully. Ask clarifying questions. Be sure you understand the legal commitment you are making.

But, Don’t be Obsessive

Some automobile owners, like me and other medical professionals, take pride in their cars. Regardless of whether you bought a new car from the showroom or bought your car used, you want to keep your vehicle looking like new for a long time.
Unfortunately, modern life is the enemy of a great-looking car. Tar and stones from roadways can wreak havoc on beautiful finishes. Other drivers can carelessly dent your car in hospital or mall parking lots, and refuse to accept responsibility for the damage. Debris flying out of trucks, birds, and other problems add to the long list of threats to your car.
The Steps
Rather than accepting dings, scrapes and grime on your car as a fact of life, follow these five steps to keep the exterior of your car looking fabulous.
1. Get Covered
Rain, snow, and sunshine can all adversely affect the exterior of your car. You can do little about the weather while driving your car, but when you get home, you can cover your car to protect its beautiful finish. Although garages offer the best protection against outside forces for your car, you might find out that you can get similar results by using a car port or a portable garage. A portable garage is a flexible cover that you can put over your vehicle to protect its exterior while not in use.
2. Paint Protection Film
Special products exist that help protect the finish of your car at all times, even while you drive. Paint protection film creates a layer of protection between the exterior surfaces of your car and the environment, so your car can withstand an array of road hazards. This type of product eliminates expensive trips to your dealer’s body shop for touchup work and preserves the resale value of your car.
3. Wash Your Car
Although a carwash can put the exterior of your car in jeopardy, it can help prevent harmful grime build up. If you care a lot for your car, you will give it a loving hand-wash, detail and wax periodically to keep its finish looking great. While you wash, you can look for new scrapes and dents that either you or your dealer can quickly repair before they become ugly and embarrassing.
4. Cautious Parking
Parking lots pose some of the most severe threats to auto exteriors. It is my pet peeve. Regardless of how carefully you park, someone else will come along and park too close to your car, giving your car a free dent. Although often minor, parking-lot damage can cost a lot to repair. Motorists these days live with the fear that a claim will cause their insurance premiums to rise, so they might not take responsibility for denting or scraping your car.
It’s time to take parking into your own hands. You can try taking up two spots when you park, making it impossible for other car doors to reach your vehicle. Also, you can park far away from other cars where most people will never park. The long walk will give you valuable health benefits, and the remote parking spot can help prevent damage to your car.
5. Common Sense
Your best defense against scrapes, dents, and grime might reside under your own hat. Common sense should tell you to avoid roads while they undergo paving line-painting work. Avoid attempting to enter narrow alleys and resist the temptation to drive up to your mailbox when you get home at the end of the day. Never drive your car near trees and bushes. Always avoid dirt or gravel roads. Also, keep your garage and carport clear of tools and other objects that can easily fall and damage your car.

What is a Cigarette?

A cigarette, also known colloquially as a fag in British English, is a narrow cylinder containing psychoactive material, usually tobacco, that is rolled into thin paper for smoking. Most cigarettes contain a “reconstituted tobacco” product known as “sheet”, which consists of “recycled stems, stalks, scraps, collected dust, and floor sweepings”, to which are added glue, chemicals and fillers; the product is then sprayed with nicotine that was extracted from the tobacco scraps, and shaped into curls.

The cigarette is ignited at one end, causing it to smolder; the resulting smoke is orally inhaled via the opposite end. Most modern cigarettes are filtered, although this does not make them safer. Cigarette manufacturers have described cigarettes as a drug administration system for the delivery of nicotine in acceptable and attractive form. Cigarettes are addictive and cause cancer, chronic obstructive pulmonary disease, heart disease, and other health problems.

But, what makes a state a great place for driving this Memorial Day weekend, 2021; especially post the corona pandemic? Is it higher gas prices, or fewer fellow travelers on the road? Or, is it higher speed limits–whether it’s on your way to a destination, or purely for the enjoyment of driving?
And, what makes other states among the worst places to drive?

Fear not! The ME-P and CarConnection looked at five sets of data to choose our best and worst states for driving. We selected the most recent data available from a range of sources–some neutral, some not so neutral. It was normalized so that it represented a percentage of change from the average, and added up the scores to determine which states earned the lowest total. According to Wikipedia, the Overton Window is the range of ideas tolerated in public discourse, also known as the window of discourse.
The term is named after political scientist Joseph P. Overton, who claimed that an idea’s political viability depends mainly on whether it falls within a range acceptable to the public, rather than on politicians’ individual preferences.
According to Overton, the window contains the range of policies that a politician can recommend without appearing too extreme to gain or keep public office in the current climate of public opinion.

One way for a retiring doctor to stretch a retirement nest egg is to relocate your retirement nest. Finding a place with a lower cost of living can include considering retirement in another country.
International Living
According to International Living, Panama is one of the best options for Americans looking for affordable living costs, good medical services, and an appealing climate. Costa Rica, Mexico, and Belize are also good possibilities.
Before you pack your sunhat and flip-flops and head for a low-cost retirement haven like Panama, however, take a look at all the factors affecting your retirement income and expenses. One of those is taxes.
Taxes
Moving out of the country does not mean your tax bill to the US government or your current state will decrease. Short of giving up your US passport, there is nothing you can do to escape paying US taxes on your income, even if you don’t live in the US. We are one of two countries worldwide—the other is Eritrea—that taxes our citizens based on both residence and citizenship.
You might assume, however, that moving out of the country would end your liability for state income taxes. That isn’t always the case. Some states still want to tax your income even though you don’t live there. According to Vincenzo Villamena in a December 2018 article for International Living magazine titled “How to Minimize Your State Tax Bill as an Expat,” it’s especially problematic if you end up returning to your old address in the state and start filing an income tax return. Eventually, he says, “the state will see the gap” and may require you to pay taxes on the missing years.
You have nothing to worry about if you live in one of the seven states with no income tax: South Dakota, Wyoming, Nevada, Washington, Texas, Florida, and Alaska. Tennessee and New Hampshire aren’t bad, either, as they don’t tax your earnings but they do tax your investment income. Most other states will let you off the hook if you submit evidence that your residence is in another country and you haven’t lived in the state for a while.
Then there are the states that won’t let go of their former residents easily. Those are California, Virginia, New Mexico, South Carolina, North Carolina, Massachusetts, and Maryland. Assuming that when you leave you will be coming back, they require that you continue to pay state tax on your income.
Solutions?
The solution to this issue takes a little financial planning and some extra time. The best way to escape paying taxes to a state you no longer live in is to move to a state with no income tax first before relocating abroad. You must prove to your old state that you have left and have no intention of ever coming back.

This means moving for real—cutting as many ties to your old state as possible and establishing as many as possible in your new state. You will want to sell your home, close bank accounts, cancel any mailing addresses, change healthcare providers and health insurance companies (including Medicare), be sure no dependents remain in the state, and register to vote and get a driver’s license in the new state. As a final good-bye you will want to notify the tax authorities that you are filing a final tax return for your last year that you lived in the state.
Assessment
In case you need a good state from which to launch your leap into expat status, consider South Dakota. Not only would my income tax-free home state let you go easily, it would welcome you back if you should decide to return to the US.

Twelve Years of Tunes Just Vanished
A Metaphor for EHRs?

According to the MIT Technology Review, MySpace has lost all the music uploaded during its first 12 years
The company’s data protection officer blamed it on a server migration, and said it had lost over 50 million songs from 14 million artists.
A while coming:
All music on MySpace (aw, remember?) from 2015 and earlier stopped working about a year ago. Originally, the company said it was working on the issue, but it has been forced to admit all the data has been lost (no, it didn’t have any backups.)
A niche issue:
Okay, most people don’t keep their only copy of a particular record on MySpace. But the fact that so much material can be lost in one fell swoop is a reminder that the internet is not an archive. If you don’t have a physical backup, files can be lost, regardless of how unlikely that might feel.

Famed Pediatric Surgeon

Recently, I was back home in the rust-belt city of Baltimore, Maryland.
Of course, I stopped by Johns Hopkins University Medical School and Hospital where I first met Ben Carson MD, the late great Helen Taussig MD and J. Alex Haller Jr., MD decades ago. I played stickball in the parking lot there, while a kid.
So, I was saddened to lean that Dr. Haller died earlier this year; at age 91.

City Lights in Baltimore, Maryland

I stopped off in Baltimore, Maryland during the recent holidays to visit current clients, prospect for new ones, do a little public speaking and promote our book [Business of Medical Practice, third edition].
Of course, we stopped by Johns Hopkins University where my sister worked, and promoted the Medical Executive-Post, as well.
Enter Christmas Street
And so, it was with great anticipation that we agreed with our host to visit Baltimore’s Christmas Street, in a section of the city known as Hampden. For 65 years, the residents of Baltimore’s 34th Street have drawn crowds from all over the world to view their display of Christmas lights.
Crowd favorites are the motorized robot, the hubcap “Christmas tree”, and “snowmen” made from bicycle tires. The eaves of houses drip with strings of lights that illuminate Nativity scenes, while glowing candy canes light the sidewalks.
And, the hot chocolate and pizza, down the street at Angelo’s Restaurant, was especially delicious on any cold wintry night.

On Men’s Health

As the fall chill sets in, you may see some men sporting scruffy faces. It could be laziness, it could be a fashion statement, or maybe it’s a way for men to look out for each other.
Why?
Many men are giving their razors November off to raise awareness and funds for men’s health. Men die on average five years earlier than women, according to the US Centers for Disease Control and Prevention.
Prostate and testicular cancer contribute to the disparity. Also, four out of five people who kill themselves are men.

No-Shave November is one organization behind the fuzzy faces. It hopes the sudden sprouting of beards will prompt conversations at work or around town about men’s health.
Assessment
No-Shave November is also asking men to donate the money they would have spent on shaving for the month; the funds will go to fight cancer.
