Nathaniel Potter MD

Halloween (also spelled Hallowe’en) is an annual holiday celebrated on October 31st.  It has roots in the Celtic festival of Samhain and the Christian holy day of All Saints.

Today, it is largely a secular celebration but some have expressed strong feelings about perceived religious overtones.

Here are two interesting and popular ME-P articles for this Halloween season.

It is a surprising and depressing fact that 7 out of 10 doctors would not recommend healthcare as a profession to their children.

  • So, what is your outlook on this?
  • Would you recommend healthcare as a profession to a family member or friend?

What is the “Gini” Index?

What it is – How it works?

[By Staff reporters]

The Gini Coefficient (also known as the Gini index or Gini ratio) is a measure of statistical dispersion intended to represent the income distribution of a nation’s residents, and is the most commonly used measure of inequality.

It is related to the Lorenz Curve and was developed by the Italian statistician and sociologist Corrado Gini and published in his 1912 paper Variability and Mutability.

Thinking Differently about DYSLEXIA

Take the “Made By Dyslexia” Pledge!

I’ve been a big fan of Bill Gates and the Microsoft Corporation since it first went public. Maybe not so much for Steve Ballmer. And, of course, very saddened by the recent death of co-founder Paul Allen https://www.paulallen.com/

But, as a physician and board certified surgeon; stock-broker, insurance agent, Registered Investment Advisor [RIA], reformed Certified Financial Planner® and Certified Medical Planner®; as well as appointed professor of economics and finance, medical educator and human being, I have never been more proud of MSFT, and CEO Satya Narayana, after learning of this new didactic initiative. Here is why?

“Microsoft + Made by Dyslexia”

Did you know that the “Microsoft + Made by Dyslexia” is helping dyslexic students thrive with technology? https://educationblog.microsoft.com/2018/10/microsoft-made-by-dyslexia-help-dyslexic-students-thrive/#oXg7GGHaHdS7wvep.99

Definition:

Dyslexia, also known as reading disorder, is characterized by trouble with reading despite normal intelligence. Different people are affected to varying degrees. Problems may include difficulties in spelling words, reading quickly, writing words, “sounding out” words in the head, pronouncing words when reading aloud and understanding what one reads. Often these difficulties are first noticed at school. When someone who previously could read loses their ability, it is known as alexia. The difficulties are involuntary and people with this disorder have a normal desire to learn.

Therefore, the ”Made By Dyslexia” pledge is for companies, teachers, professors, educators and governments to pledge to value dyslexic thinking, and to begin taking positive steps towards supporting it.

***

***

Assessment

Rest assured,  I am ashamed to say I know little about dyslexia. I am not a communication disorders or special education expert; so Mea Culpa!

Nevertheless, I urge you to  take the pledge! I have.

What is a Linguistic Tautology?

A Tautology … A Tautology … A Tautology

[By staff reporters]

The word tautology was used by the ancient Greeks to describe a statement that was asserted to be true merely by virtue of saying the same thing twice, a pejorative meaning that is still used for rhetorical tautologies. Between 1800 and 1940, the word gained new meaning in logic, and is currently used in mathematical logic to denote a certain type of propositional formula, without the pejorative connotations it originally possessed.

***

A logical tautology is very different from a rhetorical tautology. It s a statement that will evaluate as true regardless of the truth values of the components of the sentence. For example, in logic or is non exclusive, which means that if we say A or B , the statement A or B would be true if A is true, if B is true, or if both are true. The only way A or B can be false is if both A and B are false. From this we can construct an example of a logical tautology. We can say A or not A , and it will be always be true regardless of whether A is true or not. This is the case because whenever A is false, not A is true, so at least one of the two will always be true. A real world example of a logical tautology would be the car is moving or it is not moving or the door is locked or it is not locked or the girl is pregnant or she is not pregnant.

MORE: https://en.wikipedia.org/wiki/Tautology_(logic)

Assessment

Can you think of any other linguistic tautologies?

The Difference Among Pedagogy, Andragogy and Heutagogy

ON EDUCATION – The Difference Among Pedagogy, Andragogy, And Heutagogy

[By Terry Heick]

Jackie Gerstein’s passionate thinking about learning is some of my favorite to read. She is rarely pulled down by trend or fad, but is unquestionably progressive and forward-thinking in her approaches to learning and thinking about learning.

She and I also share a passion: self-directed learning. (As does the original summarizer/author of the thinking embedded in table below, Lindy, McKeown Orwin).

I’m embarrassingly interested in any kind of learning at all–formal or informal, self-directed or teacher-centered, authentic or academic. Doesn’t mean I regard them all equally, but I do see a role for almost any system or approach that can cause, support, or glorify the processes of understanding.

Gerstein’s presentation, “Education 3.0 and the Pedagogy of Mobile Learning” uses the concept of mobile learning as a spearhead into a broader discussion of how people learn–different approaches, different domains, and different technologies.

We recently shared some thinking about what “Education 3.0” might mean as well, and are nauseatingly effusive in our praise of self-directed learning (And a primer on self-directed learning here as well.)

Assessment

With the progress of technology and the rise in mobile learning, now more than ever Self-Directed Learning–or Heutagogy–isn’t just possible, but natural, and almost awkward to not use, something Gerstein capture’s thoroughly and with her characteristic passion in the presentation below.

The cover up: According to a report in the Wall Street Journal, Google discovered a bug in Google+ code in March and fixed it, but decided not to reveal its existence. An internal Google memo showed that executives were worried about the damage the news would do to Google’s reputation at a time when Facebook was under fire for mishandling customer data in the Cambridge Analytica affair.

The bug: It had been around since 2015 and was found in code that lets third-party app developers access publicly available Google+ profile data about a user and their connections, with the user’s permission. The glitch meant developers could access private details about people’s friends too, for example their email addresses, birthdays, and relationship statuses. Google has now shut off consumer access to Google+.

The consequences: The cover up will put even more pressure on policymakers to scrutinize the power of big tech companies and their grip over our data. It will also fuel attempts by privacy activists to get tougher laws in place to force companies to reveal actual and potential data leaks, such as Europe’s new data protection regime.

Term Life Insurance Can Protect Retirement Plan Contributions

Term Life Insurance Can Protect Retirement Plan Contributions

By Rick Kahler CFP®

Some of the typical reasons for life insurance are to replace a breadwinner’s salary, pay off large debts, and pay estate taxes. Another reason to carry life insurance—if it’s the right kind—can be to fund a retirement plan.

Illustration

To illustrate, let’s imagine a couple, both 55, with two grown children, two good jobs, and no debts. They began funding their retirement only recently. Leigh’s entire salary of $124,000 a year goes into company retirement plan options: $64,000 into the 401(k) and profit sharing plan and $60,000 into the Cash Balance plan. The couple lives on Mischa’s salary of $60,000 a year.

Their financial planner has calculated that in 10 years they will have a good chance of having $1,500,000 saved in retirement plans. This amount will allow both of them to retire, continue to live on $60,000 a year for the rest of their lives, and have enough to fund long-term care for one of them or leave a nice inheritance to their kids.

The death, disability, or loss of a job of either of them is not a threat to their current lifestyle. However, it is a threat to their retirement plan. And the loss of Leigh’s job is the biggest threat. While finding a new job that would allow retirement plan contributions to resume is possible, it is not guaranteed.

Nothing can be done to insure against the loss of a job, but there are ways insurance could help protect this couple. They could purchase disability insurance to replace 60% of the income of either partner. This would allow them to still make a reduced contribution to their retirement plan.

Premature Death

But what happens if either of them should die prematurely, especially Leigh? It’s doubtful Mischa could cut expenses enough to put anything significant toward retirement, instead having to work as long as possible and then rely heavily on Social Security.

This a where a 10-year term life insurance policy on Leigh would make a significant difference. If they purchased a $1,000,000 term policy on Leigh now, the premium (for a nonsmoker in good health) would be around $1200 annually. Should Leigh die this year, if Mischa invested the insurance payment it would have a high probability of growing to $1,500,000 in ten years. This would allow Mischa to retire comfortably.

However, the older Leigh and Mischa get, the less they need the insurance. If Leigh died in the fifth year of the policy, the five years of savings plus the insurance proceeds would accrue more than $2,000,000 over the following five years.

One cost-saving strategy would be to buy two $500,000 10-year term policies and drop one after five years. This would still provide for a total of around $1,500,000 in retirement funds for Mischa by age 65 if Leigh should die before that time.

***
 ***

If you proposed this plan to a life insurance agent, they might suggest putting Leigh’s salary into a cash value policy instead of buying term.

Let’s look at that

Contributing $124,000 into the retirement plan saves $24,000 a year in income taxes, so only $100,000 a year would be available to buy insurance. This amount would cover a policy with a $1.9 million death benefit and a cash value guaranteed to grow to $1,036,328 in 10 years. Given the extra tax payments, plus premium costs of $1 million over 10 years, that’s not a good investment for our couple. The commission of $72,500 makes it a great investment for the salesperson, though.

Assessment

Besides, in this circumstance, life insurance is not meant as an investment. It is an affordable way to replace the income that covers Leigh’s retirement contribution.

CERTIFIED MEDICAL PLANNER® CHARTER DESIGNATION PROGAM

[A Continuing Education Portal for Financial Advisors]

By Ann Miller RN MHA

An Information Technology Educational Futurist

Today, colleges and universities are finally beginning to identify students who are adept at learning online and reward the top achievers and professors. Employers, graduate and medical schools are beginning to troll MOOCs [massive open online courses] seeking viable job, and academic, candidates.

In fact, when I last checked, the nation’s public health administration and related B-student were enrolled in more than 118 online programs. MOOCs offer greater access for a larger number of students, at significantly lower costs than on-site programs.

By the same token, technology like Blackboard®, Cernage, and eXplorance, Kalture and related must be used to full potential. Smart phones, PCs and tablets, videos, interactive games, AI simulators and related apps with Skype®-like virtual classrooms and cloud storage are obvious embellishments to online initiatives.

An Executive Education Pioneer

Moreover, it is increasingly imperative that technology be used to expand the universe of targeted adult-learners. This is for aspiring professionals and business executives, or those already in the workforce.

Estimates by Business Week suggest that adult executive education in the US is a $900 million annual business with approximately 80 percent provided by university schools. Beside the educational benefits, monetary dividends are reaped as open enrollment eases matriculation access. Similar programs at the Wharton School, Darden, Harvard and the Goizueta Business School at Emory University charge premium rates for the implied institutional moniker.

ENTER the CERTIFIED MEDICAL PLANNER® charter designation

According to industry pundit: Mike Kitces MSFS CFP CLU ChFC EA

The CERTIFIED MEDICAL PLANNER™ charter designation program was created by Dr. David Marcinko (who edited the Financial Planning Handbook for Physicians and Advisors” [1st and 2nd editions”] AND “The Business of Medical Practice [1st, 2nd and 3rd editions]. “The informed voice of a new generation of fiduciary advisors for healthcare”

 Like medical professionals, all licensed Certified Medical Planner™ charter-holders are required to act in accordance with governing regulations. They are required to sign a Code-of-Ethics attestation confirming the intent to run their advisory and/or management consulting business according to a strict set of fiduciary standards.

PROGRESS: After several years of proof-of-concept preparation, we secured the website URL: http://www.CertifiedMedicalPlanner.org complete with copyrighted logo and launched. We now have about 60 graduates under a quarter-semester business model with 3 mandated proprietary textbooks, case models, test questions and checklists, and 3 recommended proprietary dictionary handbooks which we produced and copyrighted.

Our strategic competitive advantage [SCA] is four-fold: fiduciary status, asynchronous education with “live” instructors, deep curriculum granularity and requisite undergraduate degree.

PRODUCT LINE EXTENSION: Our course materials are kept updated thru our website platform: http://www.MedicalExecutivePost.com with half million readers / subscribers

Full Disclosure: We are currently under non-disclosure agreements [NDA] with a VC firm located in Durham, NC that acquires, invests and operates a portfolio of educational and healthcare media, market intelligence, online certification programs and associated businesses.

What is a Philosophical Razor?

Keep Calm and Use These Epistemological Principles

[By staff reporters]

According to Wikipedia, in philosophy, a razor is a principle or rule of thumb that allows one to eliminate (“shave off”) unlikely explanations for a phenomenon, or avoid unnecessary actions.

Razors include:

  • Occam’s razor: Simpler explanations are more likely to be correct. Avoid unnecessary assumptions.
  • Grice‘s razor: As a principle of parsimony, conversational implications are to be preferred over semantic context for linguistic explanations.
  • Hanlon’s razor: Never attribute to malice that which can be adequately explained by stupidity.
  • Hume‘s razor: “If the cause, assigned for any effect, be not sufficient to produce it, we must either reject that cause, or add to it such qualities as will give it a just proportion to the effect.”
  • Hitchens’s razor: “What can be asserted without evidence can be dismissed without evidence.”
  • Newton’s flaming laser sword: If something cannot be settled by experiment or observation, then it is not worthy of debate.
  • Popper’s falsifiability principle: For a theory to be considered scientific, it must be falsifiable.
  • Sagan standard: Extraordinary claims require extraordinary evidence.

Fixing the Web

Tim Berners-Lee has a plan to fix the web

[By MIT Technology Review]

The World Wide Web’s inventor, Tim Berners-Lee, has launched a way to make it easier for people to control their personal data. Will it take off?

The news:

Berners-Lee is working with people at MIT and beyond on a startup called Inrupt which runs an open source project, Solid, that hands you back control over your own data.

How will it work?

Decentralization and control are the guiding principles. Solid will let developers create decentralized apps that run on data fully owned by users. They decide where to store their data, and who to share it with. The first wave of apps being built on Solid is being developed now.

Good luck:

The plan is very ambitious. Its success will hinge on whether enough people adopt it beyond just hardcore techies. It’s well-timed in the week of Facebook’s breach though, and the fact it’s the brainchild of Berners-Lee can’t hurt either.

What is Your Teaching Philosophy?

 Here is My Teaching Philosophy

[By Dr. David Edward Marcinko MBA]

Although any learner-centered teaching philosophy, or Boyer Model of scholarship, is constantly in flux, the mission of a public or private educator is: [1] to promote positive learning; [2] to motivate students, staff and graduates; [3] to provide a strong foundation for lifelong learning; and in modernity [4] to enhance career and life-work opportunities; to [5] improve bottom-line financial metrics, and [6] to collaborate on a national and global basis.

However, because we are specifically operating in the rapidly changing healthcare, business management, investing, finance, economics and education milieu, even deeper experiential insight is needed.

Developing NEW Teaching AND Education Skills FOR Business and Healthcare 2.0

Medicine and healthcare business today is different than a generation ago, and all educators and healthcare professionals need new skills to be successful.

Traditionally, the physician – like the classroom professor – was viewed as the “captain of the ship”. Today, their role may be more akin to a ship’s navigator, utilizing clinical, teaching skills and knowledge to chart the patient’s, or student’s, course through a confusing morass of requirements, choices, rules and regulations to achieve the best attainable clinical or didactic outcomes.

This new teaching paradigm includes many classic business school principles, now modified to fit the PP-ACA, the era of health reform, and modern technical connectivity. Thus, a Professor, Chair or Dean must be a subtle guide on the side; not bombastic sage on the stage.

These, newer teaching philosophies must include:

  • Negotiation – working to optimize appropriate curricula, services and materials;
  • Team play – working in concert with others to coordinate education delivery within a clinically appropriate and cost-effective framework;
  • Working within the limits of competence – avoiding the pitfalls of the generalist teacher versus the subject matter expert that may restrict access to professors, texts and facilities by clearly acknowledging when a higher degree of didactic service is needed on behalf of the student;
  • Respecting different cultures and values – inherent in the support of the academic Principle of Autonomy is the acceptance of values that may differ from one’s own. As the US becomes more culturally heterogeneous, educators and medical providers are called upon to work within, and respect, the socio-cultural and/or spiritual framework of patients, students and their families;
  • Seeking clarity on what constitutes marginal education – within a system of finite resources; providers and professors are called upon to openly communicate with students and patients regarding access to marginal education and/or treatments.
  • Supporting evidence-based practice – educators, like healthcare providers, should utilize outcomes data to reduce variation in treatments and curriculum to achieve higher academic efficiencies and improved care delivery;
  • Fostering transparency and openness in communications – teachers and healthcare professionals should be willing, and prepared, to discuss all aspects of care and academic andragogy; especially when disclosing problems or issues that arise;
  • Exercising decision-making flexibility – treatment algorithms, templates and teaching pathways are useful tools when used within their scope; but providers and professors must have the authority to adjust the plan if circumstances warrant;
  • Becoming skilled in the art of listening and interpretingIn her ground-breaking book, Narrative Ethics: Honoring the Stories of Illness, Rita Charon, MD PhD, a professor at Columbia University, writes of the extraordinary value of using the patient’s personal story in the treatment plan. She notes that, “medicine practiced with narrative competence will more ably recognize patients and diseases; convey knowledge and regard, join humbly with colleagues, and accompany patients and their families through ordeals of illness.” In many ways, attention to narrative returns medicine full circle to the compassionate and caring foundations of the patient-physician relationship. The educational analog to this book is, The Ethics of Teaching [A Casebook], co-edited by my teacher and colleague Deborah Ware Balogh PhD of the University of Indianapolis.

Finally, these thoughts represent only a handful of examples to illustrate the myriad of new skills that tomorrow’s healthcare professionals, and modern educators, must master in order to meet their timeless professional obligations of compassionate patient care and contemporary teaching effectiveness.
