Hospitals blame reform, economy for layoffs
Cash-strapped hospitals across the country are sending out pink slips as they prepare for possible Medicare reimbursement cuts and state challenges to expanding Medicaid benefits under federal healthcare reform.

CMS Announces Participants in Primary-Care Initiative
The CMS named the 500 primary-care practices that will participate in a provider-payer coordination pilot program aimed at improving care for Medicare beneficiaries and other patients. The Comprehensive Primary Care Initiative, which involves seven federally-sponsored regional pilot sites nationwide, will offer bonus payments to participating primary-care physicians who better coordinate their patients’ care.
The participating practices are in eight states: Arkansas, Colorado, New Jersey, New York, Ohio, Kentucky, Oklahoma, and Oregon. The regions in those states were selected in April based on the percentage of the total population covered by payers who expressed interest in joining the pilot project, according to the CMS. Participating physicians will receive a care management fee, which will average $20 for each beneficiary monthly, to help cover the cost of improved service coordination for patients.

CMS Nails Down Medicaid, Medicare Raises for Primary Care
The Centers for Medicare & Medicaid Services (CMS) has nailed down raises that it had promised primary care physicians in each program next year, but they will resemble hors d’oeuvres on the Titanic if Congress does not avert a 26.5% Medicare pay cut set for January 1. Under a final Medicare physician fee schedule for 2013 released by the agency, family physicians, geriatricians, and internists will receive an overall payment increase of 7%, 5%, and 4%, respectively.
The raise mostly takes the form of a separate fee for coordinating a patient’s care for the first 30 days after leaving a hospital, skilled nursing facility, or certain outpatient services. Physicians can bill this service using 2 new billing codes, 99495 and 99496. CMS will fund this raise for primary care practitioners by reducing reimbursement for many specialists. Most of the cuts fall below 5%, although radiation oncologists and neurologists will take a 7% hit. Not surprisingly, specialty societies have been quick to protest.

Foreign-Educated and Foreign-Born Health Professionals in The U.S. Workforce
• Physicians who were educated outside the United States account for about 25 percent of the U.S. physician workforce, with the largest groups being from India, the Philippines, Pakistan, Mexico and the Dominican Republic.
• Foreign-born registered nurses account for 12 percent to 15 percent of the total RNs in the United States, with 5.4 percent both foreign born and foreign educated. The largest number of foreign-educated and foreign-educated RNs are from the Philippines, followed by Canada, India, the United Kingdom and Nigeria.
• Among direct care workers, a category that includes nursing aides and home health aides, foreign-born individuals account for 20 percent to 24 percent of the workforce. The largest numbers of foreign-born direct care workers are from Mexico, the Philippines, Jamaica, Haiti and the Dominican Republic. It is estimated that 20 percent of direct care workers are undocumented immigrants.

The Department of Health and Human Services (HHS) announced $83.4 million in Affordable Care Act funding to support primary care residency programs in 60 Teaching Health Centers across the nation. The 10 hospitals who are eligible for the highest award amount are listed below.
