Reaching for a patient chart in the ER one night, a young physician is suddenly overwhelmed by a strange mixture of panic, anger and fear. But, such emotions are unacceptable for a doctor.
A Self-Loathing Doctor -OR- Modern Realist?
Tearing down a hallway, he slips into a room and as the door clicks shut, he slumps to the floor. The truth is wrenching: he hates being a doctor. In the burial ground of his emotions he unearths the regret, guilt, confusion that he feels. Like a sudden rainstorm, years of anguish and disillusionment come pelting downward. Emotions are a dangerous mix of chemicals when buried. Forcing himself to a standing position he returns to his patients, his priority. There is no going back from a defining moment; one cannot un-ring a bell.
“Is there something wrong with me?” he wonders. It is a burning, compelling question that takes him across the globe and back to answer.

AMA Profits Up But Membership Down
Profits are up and membership is down at the American Medical Association, whose 2009 annual report listed an operating profit of $16.5 million—up 560% from $2.5 million in 2008—and a membership of 228,000, which is down almost 3.4% from the 236,000 reported the previous year.

More on the Vanishing Oath
A video making waves online this week is a clip from “The Vanishing Oath,” a film by Ryan Flesher, MD, that illustrates how overhead costs, malpractice insurance and other expenses slash physician’s take home pay from the national average of $146,000 a year to just $27.72 an hour … or roughly $58,000 a year.

Physicians have never needed to negotiate for their pay. Historically it was good enough to simply represent patients’ interests.
Modern healthcare stakeholders, including insurers, favored vendors and the US government, have effectively excluded busy physicians a place at the bargaining table, and the AMA is powerless to represent patients’ needs.
Patients need their physicians to be paid a fair wage, or their physicians will disappear.

CSC: Start planning ACOs now
Though the Patient Protection and Affordable Care Act doesn’t take full effect until 2014, right now is the perfect time for providers to start developing Accountable Care Organization [ACO] plans, according to this white-paper from Computer Sciences Corp.
