  2. 10 Facts about Population Health

Targeting, tracking and treating the less healthy members of your community will be critical to bring costs under control as health care moves toward accountable care.

For trustees, this means rethinking strategic plans and reallocating limited resources.

And, of course, an expert [and founder] of this movement is our own ME-P “thought-leader” David B. Nash MD MBA. Be sure to check out his blog: Nash on Health Policy. http://nashhealthpolicy.blogspot.com/

  3. Public Health Matters

I began my career as a podiatrist specializing in the care of the lower extremity complications of diabetes and vascular disease. I treated infections and gangrene, and often had to cut away dead tissue in the hopes of avoiding limb loss.

My patients suffered from the largely avoidable complications of preventable diseases. They were the flesh and blood evidence of the failure of a system that devotes little of its considerable resources to preventing disease—and provides more “sick care” than health care.

In frustration, I looked upstream to find the causes and re-discovered public health and population health. I learned that many people are trying to rewrite the story of health in our nation so that it begins with “happily ever after.”

More are joining us every day as the nation realizes there is not enough money in the US Treasury to treat every illness that could have been prevented.

  4. Big Data’s Impact on Population Health Management and Preventative Care

According to MeriTalk, sixty-three percent of Federal healthcare executives say Big Data will help track and manage population health more efficiently, 62 percent say Big Data will significantly improve patient care within military health and VA systems, and 60 percent say Big Data will enhance the ability to deliver preventive care.

More than half, 59 percent, of Federal executives working in agencies with a healthcare-related mission say that in five years, fulfilling their agency’s mission objectives will depend on successfully leveraging Big Data.

Federal agencies focused on healthcare research and care delivery are testing the waters today. One in three say their agency has successfully launched at least one Big Data initiative-35 percent use Big Data to improve patient care, 31 percent are reducing care costs, 28 percent are improving health outcomes, and 22 percent are increasing early detection.
